Job Summary and Qualifications
The Case Manager (CM) will help facilitate discharge needs of patients with emphasis on culture, financial considerations and family dynamics. In addition, the CM ensures high-quality, patient-centered care by managing Rehabilitative care to ensure optimum outcomes. The CM provides, coordinates, and directs care specific to the needs of each Rehab patient. The CM collaborates with the Rehab Program Director and Facility Case Management Director regarding departmental functions. The CM coordinates efforts within the Rehabilitation team.
What you will do in this role:
+ Provides initial and ongoing assessments for patients/families in preparation for discharge.
+ Assist patient/families with psychosocial, cultural and financial considerations that may impede recovery and or discharge.
+ Formulates plan for post-hospital care and educates patients and families of the patient’s needs post discharge and informs them of the alternatives available to meet those needs.
+ Refers patients who require extended care post discharge to long-term care facilities. Once an appropriate bed is located, arranges for the patient’s transfer.
+ Collaborates with community agencies to provide services to assist with housing, financial transportation, psychosocial, educational, home health, and home medical equipment needs of patients/families post discharge.
+ Collaborates with the health care team and develops treatment goals and plans interventions that assist the patient and family to resolve concerns that may arise as a result of illness.
+ Initiates patient/family conference with the health care team to achieve positive outcomes.
+ Documents case management assessments, treatment goals, interventions, outcomes and discharge plans in the patient’s medical record congruent with departmental policies.
+ Provides ongoing education for the other members of the health care team related to ethical psychosocial and discharge planning issues.
+ Educates patients and families to the patient’s needs post discharge and informs them of the alternatives available to meet those needs.
+ Maintains current knowledge of trends related to medical RN/SW health care issues through attendance and active participation in continuing education and professional organizations.
+ Maintains knowledge base for identification and reporting of potential patients with contractual adjustment needs and length of stay issues.
+ Takes a proactive role in discharge planning.
+ In consultation with RN/SW CM, applies the rules of Severity of Illness, Intensity of Service and Discharge screens/indicators consistently in evaluating medical necessity.
+ Performs admission and concurrent review for all payers and maintains legible documentation as directed by the utilization Management Plan.
+ Identifies, tracks and records alternative care days (ACDs). Intervenes to prevent or decrease ACDs as appropriate.
+ Initiates Denial and appeal process in accordance with policy.
+ Demonstrates knowledge of human growth and development, assessment, range of treatment, and care of patients appropriate to the ages of patients.
+ Facilitate weekly team meetings and family conferences as needed.
+ Completes psycho-social assessment upon admission.
+ Performs insurance updates, as requested.
What qualifications you will need:
+ Graduate of an accredited school of nursing required, BSN preferred.
+ Current Registered Nurse license in Virginia or current Multistate Registered State license required.
+ 1 year of RN experience in an acute medical setting required, 2 or more years preferred.
+ Previous case management experience or RN experience on a Rehab unit preferred.
+ Nursing Case Management Certification (CCMC) preferred.
