Center Manager – PACE Program
The Center Manager oversees daily operations of the PACE center, including the day center, rehabilitation, social work, dietary, and recreation services. This role ensures high-quality care for participants and supervises the interdisciplinary team (IDT).
Key Responsibilities:
+ Lead with Trinity Health’s Mission, Vision, and Values.
+ Foster a warm, collaborative environment across departments.
+ Provide strategic leadership, manage budgets, and ensure regulatory compliance.
+ Supervise, mentor, and support staff development.
+ Facilitate IDT meetings and care planning.
+ Collaborate on staffing, quality improvement, and participant outcomes.
+ Coordinate participant transitions and community health services.
+ Support survey readiness and corrective actions.
+ Communicate regularly with senior leadership and stakeholders.

Qualifications:
+ Bachelor’s degree in healthcare or related field.
+ 5+ years in healthcare leadership; 1+ year working with elderly or frail populations.
+ Strong leadership, communication, and organizational skills.
+ Proficiency in Microsoft Office and healthcare systems.
+ Ability to manage change, prioritize tasks, and travel up to 10%.
