    Penn Medicine is seeking a full-time, day shift Ambulatory Clinical Coordinator Pharmacy for the Warfarin Clinic at the Perelman Center for Advanced Medicine (PCAM).

    Summary:

    The Clinical Coordinator will be responsible for overseeing day-to-day clinical operations of our ambulatory care clinic team, while maintaining 40% clinical responsibilities. The Clinical Coordinator must be an effective communicator and be able to integrate themselves within the healthcare team, assist team with day-to- day clinical issues, appropriately assess and escalate any pertinent issues/questions to the pharmacy management team, improve upon existing clinical infrastructure and work toward any clinic expansion efforts. They will adhere to Pharmacy Policies and Procedures and strive to achieve organizational goals and objectives.

    Responsibilities:

    -Provide oversight and develop a strategic plan along with Associate Director of Ambulatory Pharmacy for clinical services in AMC clinic including expansion of service provision to growing areas, quality and research initiatives, publications, speaking events and clinical progression.

     

    -Primary point of contact for AMC’s day-to-day clinical operations and assist with all team member questions or clinic issues that arise including but not limited to: customer recovery, patient complaint management, clinical workflow clarification, inventory management, daily workload and redistribution strategies.

     

    -They will assist with implementation and monitoring of Collaborative Practice Agreement (CPA) compliance within the AMC clinic.

     

    -Assure clinical team members are actively engaged in the clinical program by providing ongoing education, monitoring and competency evaluation.

    -Ongoing evaluation and improvement of clinical services within AMC

    -Maintain clinical practice by devoting 40% of time to patient care activities of the AMC clinic. Clinical service activities include but not limited to: participate in all tasks and responsibilities of an AMC clinical pharmacy specialist, assist other team members with clinical activities as needed such as lab entry, patient follow up, customer service recovery, and patient complaint management.

     

    -Serve as the entity committee representative for any relevant health system meeting.

     

    -Coordinate and manage residents/students assigned to ambulatory rotations within AMC working closely with inpatient student Coordinator and Resident Program Director.

     

    -Provide cross coverage when needed for clinical services.

    Credentials:

    + Current Pharmacist License in the State of PA (required)

    Education or Equivalent Experience:

    + Bachelor of Arts or Science or Doctor of Pharmacy Degree from an accredited college of pharmacy(required)

    + Post Graduate Year 1 (PGY1) Pharmacy Residency (required)

    + Post Graduate Year 2 (PGY2) Pharmacy Residency or 3-5 years of relevant clinical experience(preferred)
