### Job Duties

The Social Worker – CCB Case Manager provides case management services to individuals and families, assisting them in accessing needed social, medical, educational, and other support services. Responsibilities include developing and reviewing MnCHOICES Support Plans, coordinating and evaluating services, monitoring service delivery and provider compliance, and performing administrative case management activities. The position also involves working collaboratively with clients, families, county staff, and community resources, using a person-centered approach to ensure quality and effective support. Additional duties may include assisting in agency operations and performing other job-related tasks as assigned.

### Minimum Education Required

A bachelor’s degree from an accredited four-year college or university in social work, psychology, sociology, or a closely related field. Alternatively, a bachelor’s degree in any field with at least one year of experience as a social worker in a public or private social services agency, or a public health nurse or registered nurse, may also qualify.

### Minimum Experience Required

At least one year of experience as a social worker in a public or private social services agency is required if the candidate’s bachelor’s degree is not in social work, psychology, or sociology.
