    RN Home Care Case Manager – Home Health

    **Location:** Springfield, Oregon

    **Schedule:** Full Time, Day shift (1.00 FTE)

    **Salary Range:** $45.42 – $71.98 per hour (based on experience, education, training, and collective bargaining agreement terms)

     

    At **PeaceHealth** , we believe in bringing care to patients where they feel most comfortable—their home. We are seeking a dedicated **RN Home Care Case Manager** to join our Home Health team. In this role, you’ll provide skilled nursing care and coordinate services that support patients and families in achieving their health goals.

    What you’ll do:

    + Perform initial evaluations and determine patient eligibility for home health services.

    + Deliver skilled nursing care and provide education to patients and caregivers based on individualized treatment plans.

    + Develop, coordinate, and manage patient-specific care plans in collaboration with physicians and the interdisciplinary team.

    + Supervise and guide Home Health Aide care plans as patient needs evolve.

    + Facilitate smooth care transitions through discharge planning and ongoing communication.

    + Document care accurately and in compliance with regulatory and agency standards.

    + Support patients and families with compassion, including bereavement services as appropriate.

    Qualifications:

    + Graduate of an accredited school of nursing; BSN preferred.

    + Current Registered Nurse license in state of practice.

    + Two years of acute clinical or home health/hospice nursing experience required.

    + One year of home health experience preferred (orientation provided to achieve competency).

    + Current Basic Life Support (BLS) certification.

    + Valid driver’s license and proof of auto insurance.

    + Strong clinical judgment, communication, and care coordination skills.
