    Job Summary and Responsibilities

     

    Are you a highly skilled and empathetic RN driven to provide holistic, life-affirming care? Do you believe in supporting individuals and their families through sacred moments? If so, we invite you to become a vital part of our dedicated hospice team!

    What You'll Do:

    + **Holistic Assessment & Planning:** Develop personalized, comprehensive plans of care through patient/family interviews, physical assessments, and psychological history.

    + **Expert Case Management:** Document skilled care, treatments, and diagnostic procedures, ensuring meticulous records.

    + **Continuous Evaluation:** Assess patient status on admission and subsequent visits, regularly inform physicians of changes, and coordinate interdisciplinary services.

    + **Compliance & Documentation:** Complete initial evaluations, recertification, discharge summaries, and all federally required forms (e.g., Medicare).

    **Shift:** 8am - 5pm

    **Core Schedule:** Monday - Friday; Includes On Call Rotation

    Job Requirements

    What You'll Bring:

    + **RN License (Oregon):** Current and in good standing.

    + **BLS Certification (AHA):** Within 30 days of hire.

    + **Reliable Transportation:** Valid Oregon Driver’s License, insurance, and own vehicle.

    + **Experience:** 2 years acute care nursing; 1 year hospice or 2 years home health nursing preferred.

    + **Education:** Associate’s Degree in Nursing.

    Pay Range

    $53.22 - $74.35 /hour
