Are you a highly organized, compassionate, and detail-oriented professional ready to make a real impact on patient care? We're seeking a **Care Coordination Assistant** to join our dedicated Clinical Care Coordination team.

This vital role supports our RN Care Coordinators and Social Workers by managing critical, time-sensitive functions that ensure seamless patient transitions and optimal outcomes. What You'll Do:

+ **Facilitate Post-Acute Care:** Manage timely referrals, communicate with agencies, and ensure follow-up appointments to prevent readmissions and mitigate denials.

+ **Patient & Family Liaison:** Directly communicate with patients and families, providing essential information and securing necessary documentation.

+ **Interdisciplinary Collaboration:** Work closely with healthcare teams, patients, and external stakeholders to progress care efficiently.

+ **Ensure Compliance:** Uphold strict confidentiality (HIPAA), deliver regulatory notifications, and identify process improvements.

+ **Administrative Support:** Manage transportation referrals, insurance requests, authorizations, and crucial communications for licensed staff.

Schedule: Monday - Friday 8am - 4:30pm

Job Requirements

What You'll Bring:

+ **Required Education/Experience:**

+ Associate's Degree (Business/Health) OR High School Diploma/GED + current healthcare professional license/certification (MA/CNA/EMT).

+ Medical terminology expertise (certificate or demonstrated knowledge).

+ Intermediate to advanced computer skills (Microsoft Office/Google Workspace).

+ **Required Skills:**

+ Excellent customer service and communication (written/interpersonal).

+ Strong analytical, problem-solving, and detail orientation.

+ Ability to manage multiple tasks independently in a fast-paced setting.

**Preferred Qualifications Welcome:** 3+ years healthcare experience, Bachelor's degree, experience in demanding patient care environments, relevant certifications.

Pay Range

$18.96 - $26.78 /hour
