Description:
This is a full time, 64 hours per pay period position. This position would work Mon-Fri one week and Wed-Fri the other week.
We are seeking a compassionate and flexible Float Social Worker to join our interdisciplinary care team. This full-time position is designed to support social work coverage across multiple medical units and the psych med unit. The Float Social Worker will float between units to help out with day to day tasks, as well as provide coverage during colleague PTO, sick calls, and other staffing gaps as needed. This role is essential in maintaining continuity of care and ensuring patients receive timely psychosocial support and discharge planning.

What You’ll Do
+ Perform thorough psychosocial assessments for patients and families
+ Develop and implement treatment plans tailored to emotional, social, and resource-related needs
+ Deliver brief, solution-focused counseling and support discharge planning
+ Advocate for patients and coordinate referrals to community services
+ Collaborate with interdisciplinary teams to ensure holistic care
+ Maintain accurate and timely documentation of all interventions and outcomes
+ Meet departmental productivity and performance standards

What You Bring
+ **Education:** Master’s Degree in Social Work or related field from an accredited institution
+ **Licensure:** Eligible for LLMSW or LMSW licensure through the Michigan Board of Social Work
+ **Experience:** Minimum of 2 years in a healthcare setting preferred
+ **Skills:**
+ Strong clinical judgment and psychosocial assessment abilities
+ Crisis intervention and problem-solving expertise
+ Empathetic communication and interpersonal skills
+ Familiarity with community resources and continuity of care principles
+ Proficiency in documentation and electronic medical records
