    Overview

     

    Westchester Jewish Community Services (WJCS), a nonprofit founded in 1943, has grown to be the largest provider of licensed outpatient community mental health services and one of the largest human service agencies in Westchester County. We provide state-of-the-art programs and services and compassionate care to more than 20,000 people of all ages and backgrounds each year.

     

    We are seeking a full-time Care Manager to work in our Certified Behavioral Health Clinic (CCBHC) in Yonkers, NY. This role will be part of a small multidisciplinary team that provides support and assistance to consumers (aged 4 throughout the lifespan). The salary for this full-time position is $50,000.

    Responsibilities

    As a valued member of our team, your responsibilities will include, but are not limited to:

    + Enrolling individuals into wrap around services, providing initial engagement and assessment for COPS Referrals and New Clients entering services.

    + Completion of initial and ongoing NOMS assessments.

    + Conducting Social Determinants of Health assessments.

    + Collaborating with clients & clinicians to create care plans, tailored to the individual.

    + Coordinating with care team & following up on referrals.

    + Primary care management.

    + Participating in multidisciplinary care planning meetings & grand rounds.

    + Assisting the team with crisis intervention and stabilization, as needed, to support the client.

    + Participating in outreach and recruitment activities with the team, forging relationships with community providers to help individuals access care.

    Requirements:

    We are seeking candidates who possess the following qualifications:

    + Bachelor’s degree in Social Work or related field

    + Bilingual English/Spanish is preferred

    + Minimum of 1 year of relevant experience

    + Excellent interpersonal, engagement, and communication skills
