ESSENTIAL FUNCTIONS:
+ Responsible for maintaining a system of supervisory service to residents, through establishment and implementation of policies and procedures, which will be followed on all shifts.
+ Directly supervise all Home Health Aides in the Licensed Home Care Agency Program.
+ Maintaining knowledge of general whereabouts of each resident.
+ Monitoring residents to identify abrupt or progressive changes in behavior or appearance which may signify the need for assessment and service.
+ Monitoring and guidance to assist residents in performing basic activities of daily living, including but not limited to:
+ Attendance at meals and maintenance of appropriate nutritional intake.
+ Performance of personal hygiene and grooming activities.
+ Surveillance of facility and activities of residents and staff to protect residents from harm to people and property.
+ Handling individual emergencies, or need of assistance, including arranging for medical or other services.
+ Investigation of incidents involving resident endangerment and/or injury.
+ Coordinate and assure that information regarding changes in residents’ conditions affecting their need for personal and emotional care, need for supervision or occurrence of incidents is communicated and available on an ongoing basis for all shifts and disciplines.
+ Always maintains records relating to resident care as required including but not limited to, resident communication book, medex, and medication records, resident sign out sheets, incident reports (resident, employee, volunteer) monthly weights, etc.
+ Is directly involved in daily medication management and works closely with Pharmacy to ensure medication is readily available.
+ Is responsible for overseeing the proper storage and recording of medications.
+ Communicates openly and provides Administrator with significant information regarding the day-to-day operation of your department.
+ Is directly responsible for supervising all the specific tasks of HHA’s carried out daily.
+ Adhere to confidentiality policy regarding information concerning residents and co-workers.
+ To remain on call to respond to and/or to make available 24-hour RN assistance for the Assisted Living Program. Will alternate weekend call with the Administrator.
+ Will be responsible to make sure schedule is adhered to and must handle situations that may arise from call-in’s and when necessary, fill vacancies by working the vacant shift.
+ To ensure staffing is always at proper level. And never allowing any shift to have less than the required minimum staffing.
+ Promote Assisted Living Program and Homecare in the community by holding in-services and providing marketing materials on an ongoing basis.
+ Assess referrals to the Assisted Living/Homecare Programs for appropriateness prior to admission.
+ Complete functional/social assessments with EDDY 23. Provide Case Management services to all ALP residents.
+ Develop care plans.
+ Complete clinicals and 6-month evals
+ Medicaid re-certifications.
Education & Experience
+ Graduate of a state approved school.
+ Is currently registered as an RN in New York State.
+ Has a workable knowledge of geriatrics and medications which includes, but is not limited to, their use, dosage, side effects and therapeutic effects and routes of administration.
+ Has a minimum of two years’ experience in the field of nursing and geriatrics as a practicing nurse.
