The **UW Medical Center-Montlake Pharmacy Department** is seeking a highly-motivated **Clinical Pharmacist - Inpatient.**
WORK SCHEDULE
+ 40 hours per week
+ Rotating/Variable Shift
POSITION HIGHLIGHTS
+ Provide pharmaceutical care for our Oncology Pharmacy Satellite services, including pharmacotherapeutic recommendations to providers, nursing, and other members of the health care team to optimize patient care
+ Responsibilities include, but are not limited to direct patient care, drug dispensing, research & education, and practice management as it pertains to the care of the assigned patient population
PRIMARY JOB RESPONSIBILITIES
+ Ensure appropriate drug therapy and nutrition, including indication, dose, drug interactions, and drug allergies.
+ Evaluate pharmacotherapeutic regimens for effectiveness and adverse drug events, and intervene as needed.
+ Utilize the medical record and other appropriate documentation systems to communicate with healthcare professionals and to document specific pharmacotherapeutic recommendations.
+ Provide drug information and education to medical teams, nursing, and dietitian staff.
+ Provide patient/family counseling and education. Update patient medication lists and reconcile medication orders upon hospital admission and discharge, as well as during clinic visits.
+ Perform comprehensive medication therapy review and other activities as necessary to ensure safe transitions of care between the inpatient and ambulatory settings.
+ Monitor and modify patient medication therapy in collaboration with the medical team to achieve desired therapeutic outcomes in the ambulatory setting
+ Provide primary preceptorship of pharmacy residents and student pharmacists.
+ Perform the duties of a Pharmacist 2
REQUIRED POSITION QUALIFICATIONS
Two years of experience in pharmacy practice. Months spent in the completion of an American Society of Health System Pharmacists accredited residency training program may substitute month-for-month for the experience.
LEGAL REQUIREMENT
State of Washington Pharmacist License.

Compensation, Benefits and Position Details
Pay Range Minimum:
$151,716.00 annual
Pay Range Maximum:
$217,092.00 annual
Other Compensation:
Night Shift Premium - $8.00/hour; Evening Shift Premium - $1.75/hour; Weekend Shift Premium - $1.50/hour
