**Schedule:** Full Time

**Shift** : Monday- Friday 8am-5pm.

**Location:** Travel with home health to patients home within the Kansas and Missouri Territory. Looking mostly for Central to North Missouri locations

The role you'll contribute:

Carries out various physical therapy procedures under the supervision of a Physical Therapist. State licensure required in Kansas and Missouri.

The value you'll bring to the team:

+ Provides treatment to relieve pain and to develop or restore function as ordered on Plan of Care.

+ Provides accurate documentation.

+ Teaches use of assistive devices and equipment to patients and caregivers.

+ Observes, records and reports to physician any untoward reactions to treatment.

+ Instructs and supervises family members to provide continuous therapy and home treatment plan.

+ Maintains open communication with case manager.

The expertise and experiences you'll need to succeed:

Minimum qualifications:

+ Graduated from a physical therapist assistant curriculum approved by the Commission on Accreditation in Physical Therapy Education of the American Physical Therapy Association.

+ if educated outside the United States or trained in the United States military, graduated from an education program determined to be substantially equivalent to physical therapist assistant entry level education in the United States by a credentials evaluation organization approved by the American Physical Therapy Association or identified at 8 CFR 212.15(e).

+ Passed a national examination for physical therapist assistants.

+ Registered Nurse - State Licensure and/or Compact State Licensure Currently licensed and registered in the state of Kansas and Missouri

+ Active American Heart Association BLS

+ Valid in state Drivers License with current car insurance

Preferred qualifications:

+ 1 year of institutional experience

+ 1 year home health experience
