Salary: $1188 / Week

LPN - Post Acute Rehab

Requirement: At least 1 year experience in rehab, medsurg, LTC, or skilled nursing

Schedule: Full-time; 36 hours/week

Hours: 12hr shifts; 7-7

This role supports the nursing team in delivering high-quality, patient-centered care to individuals recovering from serious medical conditions such as strokes, orthopedic surgeries, spinal cord injuries, and neurological disorders. The LPN plays a vital role in promoting recovery and independence.

Essential Duties and Responsibilities:

+ Collaborate with therapists to help patients perform activities of daily living (ADLs).

+ Facilitate rehabilitation exercises and routines as outlined in the care plan

+ Provide basic education on self-care practices.

+ Foster a supportive and encouraging environment for recovery

+ Accurately record patient care activities, observations, and responses to treatment.

+ Report changes in patient condition to the RN
