RESPONSIBILITIES

I. JOB SUMMARY/RESPONSIBILITIES:
• Under the general supervision of the Quality & Patient Safety Manager, is responsible for competency in coordinating and performing day-to-day operations, providing concurrent and/or retrospective review, and improving documentation of all disciplines, conditions, treatments, and care plans to ensure highest quality of care is provided to the patient.
II. TYPICAL PHYSICAL DEMANDS:
• Sitting, finger dexterity, seeing, hearing, speaking.
• Constant: reaching above, at and below shoulders; repetitive arm/hand motion.
• Frequent: standing, walking, stooping/bending, gripping of objects, and twisting body.
• Infrequent: lifting and carrying items up to usual weight of 10 pounds.
III. TYPICAL WORKING CONDITIONS:
• Not substantially subjected to adverse environmental conditions.
IV. MINIMUM QUALIFICATIONS:
A. EDUCATION/CERTIFICATION AND LICENSURE:
• Current Hawaii State license as a Registered Nurse.
• Associate degree in nursing or higher.
• Valid U.S. driver’s license, vehicle and vehicle insurance as required by Hawaii State law.
• Driver’s abstract or evidence of good driving record.
• Current BLS certification.
B. EXPERIENCE:
• Minimum one (1) year experience in health care as a clinician (RN).
• One (1) year experience in a Medicare Certified home health agency.
• Working knowledge of OASIS; certification preferred. (COS-C or COQS)
• Working knowledge of Medicare Certified Home Health financial system.
• Experience to demonstrate:
o Ability to apply decision making and problem-solving techniques to the case management process.
o Ability to interpret clinical information and assess implications for treatment.
o Ability to interpret federal, state, and other regulatory/credentialing agency requirements.
o Ability to maintain effective working relationships with clients, co-workers, and public through constructive feedback and problem-solving skills.
o Excellent time management and organizational skills.
o Ability to adapt to changing organizational needs.
o Demonstrated ability to motivate and educate others.
o Detail oriented and able to work independently to meet deadlines.
o Working knowledge of computer systems including MS Office, Outlook, Word, and Excel.
