Program Description
Pathways to Hope is a specialized program offered through WellSpan Philhaven, designed to support individuals experiencing a first episode of psychosis. Grounded in evidence-based practices, this program provides early, intensive, and coordinated care to help individuals stabilize, recover, and move forward with their lives.
The goal of Pathways to Hope is to reduce the long-term impact of psychosis by promoting early engagement, personalized treatment, and community integration. Our multidisciplinary team partners with individuals and their families to support wellness, resilience, and hope from the very first step.
General Summary
Ensures the quality of clinical care being provided by outpatient therapists in assigned clinic. Oversees the implementation of policies and procedures related to clinical care. Assists as needed with program operations, budgeting and supplies management, outcomes and other data collection, the management and oversight of office needs, support needs and client care documentation/medical records. Provides clinical oversight to the clinic/program and to other staff and provides direct care services.
Duties and Responsibilities
Essential Functions:
+ Assists in providing clinical training support and education for staff.
+ Assists in developing training programs designed to address mental health needs.
+ Provides psychotherapy to patients and their families.
+ Maintains a high level of clinical productivity of clinicians measured by target revenue goals.
+ Assists in the orientation of new clinicians and physicians and facilitates their participation in programming and services
+ Works in collaboration with supervisors to provide specific and targeted clinical supervision to increase staff therapeutic effectiveness.
+ Assists with development and implementation of program, Performance Initiatives and outcomes.
+ Assists in monitoring and reviewing charts, ensuring completeness and quality of documentation, as well as ensuring treatment plan addresses all clinical and relevant medical symptoms.
+ Assists in carrying out preparations for clinical audits and reviews, including those related to the Joint Commission and DHS.
+ Actively participates and provides feedback for the development and growth of the level of care.
+ Attends scheduled meetings related to administrative issues or organizational needs. Participates as a member of organizational committees or task groups as needed.
+ Completes Readiness Rounds and other reports, as assigned, on an ongoing basis.
+ Closely tracks sessions delivered and sessions authorized to avoid unauthorized services being delivered. Obtains reauthorizations when needed.
+ Performs annual evaluations and competency assessments of staff in accordance to policy and supports ongoing development and education of staff.
+ Responds to complaints and grievances to ensure high quality of customer service.
+ Assists in interviewing, hiring, and promotions for staff, as available.
+ Participates in targeted community outreach related to the development and/or ongoing functioning of new programs as needed.
+ Works with assigned staff to develop programming to respond to opportunities to provide services as requested.
+ Provides and/or coordinates the provision of new services including group, individual, and family therapy as well as needed case management per Best Practices standards.

+ Completes required education and competencies in a timely manner.
+ Contributes to a safe work environment through knowledge and practice of behavioral safe physical management practices, fire safety, physical plan safety, safe emergency practices, and universal precautions for body fluids.
+ Provides outstanding service to all customers; fosters teamwork; and practices fiscal responsibility through improvement and innovation.
+ Educates clients, family, and significant others regarding psychiatric diagnoses, relapse prevention, community supports, and stress management.
+ Serves as a liaison to other staff in the system providing assistance and information, when called upon to
+ address issues in Behavioral Health within the system
+ Demonstrates the use of de-escalation, mediation, and alternatives when intervening with clients and is able to teach these skills to other staff.
+ Coordinates referral process, discharges, and assists in communicating transfers to other levels of care.
+ Under the supervision of a Supervising Clinician/Provider, educates clients, family, and significant others regarding psychiatric diagnoses, relapse prevention, community supports, and stress management.
+ Provides guidance and support to other staff about clinical assessments, referrals and interventions appropriate to each individual case.
+ Conducts assessment interviews to determine client problems and needs, and initiates, develops, evaluates, and modifies the treatment plan based on those needs.
+ Ensures continuity of care through close follow-up on all cases until a reasonable level of problem resolution is accomplished.
+ Provides virtual clinical assessment and consultations as needed.
+ Develops relationships with community agencies, groups, professional affiliations as appropriate.
+ Supports the team in responses to requests from the Emergency Room or Outpatient offices for assistance in evaluating individuals with mental health concerns. This includes providing information to the Psychiatrist, consulting with other Behavioral Health Providers, completing clinical reviews with insurance companies facilitating placement to other facilities, arranging Outpatient appointments, and facilitating admissions based on assessment data as needed.
Qualifications
Minimum Education:
+ Masters Degree Required
Work Experience:
+ 2 years Clinical experience working with the full range of ages and diagnoses. Required
Licenses:
+ Licensed Clinical Social Worker Upon Hire Preferred or
+ Licensed Marriage and Family Therapist Upon Hire Preferred or
+ Licensed Professional Counselor Upon Hire Preferred
Knowledge, Skills, and Abilities:
+ Excellent interpersonal/communications skills to build working alliance, empathize with, and respond to referral sources, patients, their families, and co-workers.
+ Ability to teach staff evidenced based skills.
+ Ability to write training curriculums and assist with other administrative tasks.
+ Analytical ability necessary to manage several tasks simultaneously in a fast-paced mental health environment.
