    Summary:

    The HIV APP will work closely with ID Attending physicians, ID fellows, ID APPs, pharmacists, primary care teams, case managers, social workers, care navigators, nurses, psychologist, and other support staff to provide care to patients living with HIV or receiving HIV prevention services including HIV PrEP & PEP in the MacGregor Infection Medication and Travel Program outpatient clinic. This position will be primarily outpatient based but could also include inpatient service time, if the candidate is interested in this. This job entails seeing patients in the office, performing comprehensive assessments, providing treatment/prevention of HIV, sexually transmitted infections (STIs), and opportunistic infections, diagnosing infectious and related diseases, ordering and interpreting diagnostic and laboratory tests, prescribing pharmacologic and non-pharmacologic treatments in the direct management of acute and chronic illness and disease. Treatment plans are documented as progress notes in the electronic medical records system. Obtains accurate medical and physical history on patients and updates charts accordingly. Provides and monitors continuity of care between patient visits. Evaluation and management of emergency situations. Assists with joint Commission/Regulatory/Environment of Care Compliance measures related to the clinical staff in the office. Provides guidance and precepts for NP students. Attends weekly ID teaching and patient management conferences. The APP will also participate in the operations of the Infectious Disease clinics, sharing responsibility with two additional ID APPs with regard to providing onsite clinical support for other ID providers as needed (i.e. Seeing urgent patients followed by an ID faculty member or fellow if the patient has an urgent need and the ID faculty member/fellow is on service or out of the office, handling urgent requests for medication refills or paperwork, reviewing abnormal labs and liaising with ID faculty members/fellows to ensure optimal care of all HIV patients in the practice).

    Responsibilities:

    + Provides direct clinical care for patient population according to established protocols and utilizes evidence based practice standards in collaboration with team.

    + Maintains accurate and confidential medical records. Documents all medical evaluations, diagnoses, procedures, treatments, outcomes, education, referrals, and consultations consistent with organizational standards in the electronic health record (EHR).

    + Identifies self properly to the patient and family, and clarifies the role of the Nurse Practitioner/Physician Assistant if necessary.

    + The Nurse Practitioner/Physician Assistant should function within their scope and refer to the collaborating physician when outside of scope, and/or directed by the collaborating physician or when requested by the patient and/or family. Performs interventions specific to assigned patient population and collaborative agreement.

    + Participates in the performance improvement processes to meet quality of care and service standards.

    + Demonstrates awareness of professional responsibility as role model to peers and members of the health care team.

    + Perform approved therapeutic or diagnostic procedures based upon patient’s clinical status.

    + Addresses patient and family concerns and creates a therapeutic relationship to provide support.

    + Provide patient education, counseling and instruction regarding patient diagnosis, medical treatments, medications, and preventative care.

    + Participate in and support accreditation, compliance and regulatory activities of the organization.

    + Precept students and other health care learners.

    + Identifies patient/family needs for community resources and refers appropriately.

    Credentials:

    + BLS/CPR, as a healthcare provider as per the American Heart Association (For providers in the ambulatory setting) Required

    + Board Certified Nurse Practitioner/Physician Assistant (Required)

    + PA and/or NJ CRNP/PA-C licensure required.

    + PA and/or NJ Prescriptive Authority required.

    + PA and/or NJ RN licensure (RN) required.

    Education or Equivalent Experience:

    + Bachelor of Science in Nursing (Required)

    + Master of Science in Nursing or Health Sciences (Required)

    + New Graduate or more than 2.5 years of experience considered

    + The CRNP/PA-C must be reviewed by the Credentials Committee and approved by the Medical Board. National Board Certification required.

    + HIV Experience (Preferred)
