EAC Network, a leading not-for-profit agency dedicated to empowering and supporting over 70,000 people through 120 impactful programs across Long Island and NYC, is seeking a motivatedProgram Coordinatorfor ourSenior Services, In-Home Respite Servicesprogram.

     

    + Wage: $30 to $33 per hour

    + Schedule: Monday-Friday 9AM-5PM

    + Location: 99 Quentin Roosevelt Blvd in Garden City, NY

    Primary Purpose of Job:

    The Program Coordinator is responsible for the day-to-day operations of the In-home Respite Services Program.

    Principal Duties & Responsibilities:

    1. Client intake and case management

    + Record intakes. Provide information, guidance, and referrals to caregivers.

    + Schedule home assessments. Conduct initial home visits and re-assessments.

    + Maintain a caseload. Monitor participation, progress, and service plans.

    + Maintain complete and timely case documentation and client records.

    + Make referrals for entitlements and community services to support caregivers of older adults.

    2. Program operations and compliance

    + Prepare and maintain program records, statistics, and evaluations.

    + Prepare required reports for EAC Network and all funding sources.

    + Monitor and record collection of client fees and maintain monthly fee records.

    + Review and submit staff timesheets accurately and on time.

    3. Workforce, outreach, and training

    + Recruit, screen, and train Respite Companions and prospective volunteers.

    + Conduct program outreach to caregivers, community partners, and referral sources.

    4. Coordination and leadership

    + Coordinate schedules for home assessors, companions, and visits to ensure coverage.

    + Provide training and day-to-day guidance to Home Assessors and Senior Respite Companions.

    + Support quality, safety, and customer service standards during operating hours.

    + Escalate performance or disciplinary matters to supervision for action.

    + Perform all other relevant duties as determined by Supervision and EAC NETWORK Administration.

     

    This Role Supervises: Home Assessors and Senior Respite Companions

    Knowledge, Skills, & Abilities Required:

    + Bachelors Degree in Human Services or related field required.

    + Minimum of three years of relevant experience working with the elderly required.

    + Excellent communication, organizational, interpersonal and computer skills.

    + Must be proficient in computer applications or other automated systems such as spreadsheets, Microsoft Office applications, calendar, and email and/or database software necessary to perform work assignments.

    + Must have access to automobile and valid drivers license to operate in NY State.
