Job Description
General Description
The Care Facilitator is responsible for ensuring participants receive compassionate, timely, and appropriate care tailored to their individual needs. This role emphasizes working with people who use drugs and connecting them to health care and other supportive resources. The Care Facilitator serves as a key point of contact between participants, research team, and service providers.
Other Duties
+ Co-facilitate visits between participants and service providers
+ Monitor participants progress, and advocate for participants needs
+ Facilitate participant-centered Hepatitis C education and risk reduction training
+ Follow-up with participants and provide appointment reminders via phone, in-person, text, or email
+ Provide ongoing support and care coordination to patients throughout HCV care
+ Provide linkage to harm reduction and drug-use related services as desired by the participant
+ Attend weekly meetings
+ Maintain accurate and timely documentation
+ Collaborate with academic teams and community-based organizations
+ Ensure all services align with principles of harm reduction, and participants autonomy
+ Other duties as assigned
Qualifications
+ Bachelor's Degree
+ Minimum 2 years’ experience in care coordination, case management, or outreach
+ Experience working with HIV, HCV positive, substance use and/or high-risk populations preferred
+ Direct experience working with people who use drugs and familiarity with harm reduction principles and practices
+ Strong interpersonal, organizational, and communication skills
+ Ability to work effectively with diverse populations, including, people experiencing homelessness, and those impacted by systemic inequities
+ Ability to work a flexible schedule
+ Proficiency in Microsoft Office
+ Willingness to engage and learn from real-world experiences, and contribute to the collective effort to enhance Hep C care to people who use drugs

These functions include legal assessment and signing of agreements where RFCUNY is named as a fiscal agent; setting up award accounts; preparing sub-awards and assisting PIs in monitoring the work of the recipients of sub-awards; supporting project directors with hiring and managing research project and sponsored program staff; supporting the purchasing and paying for goods and services with grant and program funds; managing financial aspects of projects including accounts receivable, financial reporting, invoicing, budget monitoring, and cost compliance with uniform guidance; ensuring that sponsor financial requirements are met; monitoring compliance with applicable project and financial management rules and laws; supporting the management of independent and external audits and financial reviews; and providing data, information, management expertise, and other supports to CUNY’s research and sponsored programs.
