Join UPMC Hamot in Erie, PA as a full-time Pharmacist and become part of a progressive pharmacy team in a dynamic teaching hospital environment. This role offers the opportunity to work closely with interdisciplinary teams to deliver innovative, patient-centered care. Pharmacists at UPMC Hamot play a vital role in expanding clinical services and optimizing medication therapy through collaboration, education, and leadership.
You'll primarily work variable day (7:00 AM - 3:30 PM) and evening (12:30 PM - 9:00 PM) shifts, with occasional night coverage depending on departmental needs. Every third weekend and rotating holidays are required. Some scheduling flexibility may be considered. Preferred candidates will bring experience in acute care settings, proficiency with EPIC EHR, and strong communication and problem-solving skills.

In this role, there is opportunity to -
+ Interpret and verify medication orders for accuracy, completeness, and clinical appropriateness.
+ Prepare and dispense medications using safe, sterile techniques and approved technologies.
+ Review patient profiles to identify drug interactions, allergies, duplications, and therapy optimization opportunities.
+ Support decentralized pharmacy services by collaborating directly with providers and nursing on clinical care units.
+ Communicate with healthcare professionals to clarify medication orders and ensure optimal pharmacotherapy.
+ Contribute to formulary management by promoting cost-effective drug use and applying therapeutic interchange protocols.
+ Document and report adverse drug reactions and participate in medication safety initiatives.
+ Supervise and mentor pharmacy staff, students, and residents in daily operations and clinical education.
+ Participate in quality improvement projects, data analysis, and clinical pharmacy service enhancements.
+ Provide drug information and education to staff and patients, supporting evidence-based, patient-focused care.
Required
+ Graduate of an ACPE accredited College of Pharmacy
+ B.S in Pharmacy or Doctor of Pharmacy (PharmD) degree
+ Current licensure as a Pharmacist in practicing state
