Supplemental Day Home Health Supervisor - weekend coverage 0830-1700

Job Summary:

We are seeking a dedicated and experienced Home Care Supervisor to lead our large progressive Home Health Agency which serves a diverse client base throughout the Sacramento Service area. As a Supervisor, you will lead strong multidisciplinary clinical teams and play a vital role in ensuring the delivery of high-quality patient care.

Responsibilities:

+ Supervise and motivate an interdisciplinary team of home care professionals.

+ Act as a clinical resource, screening referrals and determining patient acuity levels.

+ Monitor staff performance, conduct evaluations, and implement corrective actions as needed.

+ Ensure compliance with all state, federal, and accreditation requirements.

+ Collaborate with physicians, interdisciplinary teams, and community resources to optimize patient care.

+ Assist in the development and implementation of patient/family educational materials.

+ Stay abreast of healthcare trends and recommend changes to promote quality and competitiveness.

+ Act as the MHH/MH administrator on call as scheduled.

+ Assist in providing in-service training and orientation for agency staff.

+ Represent the agency at internal and external meetings.

+ Supervise all clinical services and meet department-specific metrics to promote quality care and control operational costs.

+ Assist the department leader and Operations team with achieving and maintaining compliance with quality improvement, risk management, and utilization review strategies.

Job Requirements

Minimum:

+ Minimum of two (2) years of management/leadership experience (within the last 5 years) in a home health agency, primary care clinic or health facility, sufficient background knowledge and expertise in clinical decision making for the patient population assigned.

+ Current CA RN licensure and AHA BLS

+ **Weekend coverage 0830-1700**

Pay Range

$57.37 - $85.33 /hour
