    Job Description

    The Community Outreach Coordinator is responsible for facilitating and supporting community engagement strategies that advance public health and tobacco prevention efforts with a focus on priority populations. This role will work under The Quit Alliance, Cedars-Sinai Cancer Community Outreach and Engagement’s tobacco prevention program that houses both County- and State-funded initiatives. The Community Outreach Coordinator will lead outreach activities, strengthen collaborations with community partners, and provide technical assistance to advance health equity across these projects.

     

    The Coordinator is responsible for planning, coordinating and conducting a variety of outreach activities for the program. Will work closely with leadership to implement and achieve the goals and mission of the program. The Coordinator will ensure the timely completion of work assignments in accordance with established timelines.

    Primary Duties and Responsibilities

    + Develop and implement community engagement strategies tailored to priority populations.

    + Conduct policy reviews, assist with policy implementation planning, and participate in advocacy campaigns.

    + Plans, coordinates and performs various outreach activities including, but not limited to, site visits to various communities to interface with community residents for building awareness of organization's services.

    + Serve as a representative of the organization and/or the program to patients, families and community organizations by attending community meetings, making presentations on behalf of program and/or organization and through other community events.

    + Develop and distribute monthly calendars, newsletters and/or other outreach publications/materials.

    + Interface with community and/or education leaders in the distribution of program/organization materials.

    + Participate in the generation of reports, including program data, dashboards, outreach activities, and community contracts.

    + In conjunction with leadership and program staff; interfaces with community agencies for developing and collaborating in new or existing partnerships.

    + If conducting outreach activities for mobile clinics: Conducts patient registration at site, enter relevant patient records into electronic health records, submit accurate patient demographic information to appropriate internal registration/admission staff and schedule appointments and follow-ups.

    + In collaboration with staff, participate in development and implementation of patient/family health education.

    Department-Specific Responsibilities

    + Oversee all front office administrative functions for the mobile clinic, including patient registration, intake, appointment scheduling in CS-Link, and provider template management. Manages provider schedule templates in CS-Link and addresses appointment requests via both the platform and phone line. Support program success by ensuring a smooth clinic experience and contributing to outreach and patient access goals ensuring all work assignments are completed within established timelines.

    + Work closely with the medical team to ensure smooth clinic operations, coordinate with other staff to organize and prepare for clinic visits and assist in managing front office patient flow at the mobile clinic site.

    + Support outreach by coordinating site visits. Distribute program materials, interact with residents, and help raise awareness of clinic services

    + Maintain accurate patient records, assist with reporting and tracking outreach activities.

    + Participates actively in departmental huddles and staff meetings, occasionally leading discussions or presentations.

    + Drive to and from site visits and maintain the mobile medical unit (MMU), including daily setup/breakdown, securing parking and site logistics, overseeing vehicle cleanliness and readiness, and ensuring equipment is stored and maintained properly.

    + Assist in ensuring that the equipment undergoes regular preventive maintenance by clinical engineering.

    + Assists Clinical Nurse with monitoring and maintaining inventory of supplies and medications.

    + Transports sealed laboratory specimens (blood, urine, etc.) from MMU to CSMC lab at end of each site day.

    Qualifications

    Requirements:

    + High School Diploma/GED.

    + 2 years Experience participating in community outreach activities.
