Job Summary

The Patient Transportation Coordinator arranges and manages patient transportation to and from LaSante Health Center, ensuring accessibility for all patients, including those with specialized needs. This role manages online appointment bookings, oversees telehealth scheduling, and ensures all related documentation is completed. The Coordinator also verifies insurance coverage and maintains accurate, timely scheduling to support coordinated patient care.

Key Responsibilities

+ Schedule and coordinate transportation for patient appointments.

+ Manage online appointment bookings and telehealth schedules to ensure accuracy, avoid conflicts, and confirm required documentation is completed prior to the visit.

+ Verify patient insurance eligibility and schedule transportation accordingly.

+ Assess patient mobility or other limitations to determine specialized transportation needs.

+ Document all arrangements and special instructions in the electronic system.

+ Notify patients and staff of confirmed transportation and appointment details.

+ Coordinate interpreter services when needed for patient communication.

+ Track and report transportation requests for operational improvement.
