Job Details
RESPONSIBILITIES AND COMPETENCIES
+ Establishes, manages, and evaluates departmental policies and procedures, departmental goals and staffing levels and assignments.
+ Develops and monitors the department operational budget.
+ Interviews, hires and oversees the orientation and education of staff.
+ Develops a system of ongoing education and development of staff as a group and individually and monitors staff satisfaction.
+ Assures compliance with regulatory and external review agencies.
+ Participates in the Utilization Management Committee, reporting data on utilization trends, resource utilization and denials.
+ Serves as a resource to physicians, physician advisors, CM department, and hospital staff.
+ Collaborates with all disciplines and departments to optimize key clinical and financial outcomes.
+ Participates in committee work at SFH and its affiliates.
+ Presents the executive team with ongoing, reliable information and data as requested.
+ Facilitates patient flow through to positively impact the continuum of care.
+ Monitors/Participates in efforts to decrease LOS on an ongoing basis.
+ Collaborates with interdisciplinary team to assure medical necessity and appropriate patient status.
+ Encourages performance excellence by providing ongoing coaching and timely feedback.
+ Provides recognition and rewards the achievements of other on an ongoing basis.
+ Empowers others by effectively delegating responsibility for tasks and projects.
+ Establishes clear responsibilities and expectations with his/her staff.
+ Monitors employee performance against measurable outcomes and objectives.
+ Looks for the underlying cause of an issue to identify long term solutions that prevent recurring problems.
Communication
+ Maintains open communication with all appropriate parties.
+ Utilizes advanced conflict resolution and negotiation skills to ensure timely resolution of issues.
+ Refers cases and issues to Physician Advisor in compliance with identified procedures and follow-up as indicated.
+ Identifies, plans and coordinates aspects of care for the patient/family.
+ Initiates required forms and or letters to comply with regulatory guidelines.
+ Completes timely and appropriate documentation.

Position Requirements and Qualifications
Education/Experience
+ N. License required.
+ BSN Bachelor of Science in Nursing required
+ Master Degree required
+ Approved National Certification.
+ Demonstrates a commitment to professional development. Required to pursue ongoing education and self development to remain current with regulatory and industry standards and business objectives related to Care Management.
+ Leadership and management experience in Care Management.
Skills
+ Basis knowledge of personal computer and software for word processing and spreadsheets and/or Hospital Information Systems preferred, or the ability to enter and retrieve data from relevant computer systems.
+ Ability to effectively communicate with all levels of hospital staff in a verbal and written manner; demonstrated ability to be organized and efficient in prioritizing and managing assignments with minimal oversight and direction.
+ Demonstrates a courteous, professional demeanor and team spirit and the ability to work in a
+ Collaborative, effective manner.
+ Ability to utilize critical thinking and apply sound clinical judgment and assessment skills for decision making.
Knowledge
+ Knowledge of Federal, State and regulations preferred.
+ Knowledge of acute care, home care, sub acute care, long-term care, hospice care and rehab options and requirements.
+ Maintains knowledge of requirements by third party payers, regulatory agencies, and managed care entities concerning levels of care, continuity of benefits and medical necessity guidelines.
