Temp to Perm position after 520 hours
Days (7a-7p) - Every other weekend required
Orientation: 11/11-11/21 (8a-5p) - Tues-Friday and then Monday-Friday
Amergis Healthcare Staffing is seeking several Licensed Nurses (LPNs) to work in an intellectual and developmental disability facility in Columbia, SC
The LPNs assists individuals with intellectual disabilities or mental health challenges with growth and development of interpersonal skills and Activities of Daily Living in the home, community or residential setting. The LPN works directly with the resident and provides daily medication. The LPN should follow the guidance of the resident/ patients Individual Service Plan.
Essential Duties and Responsibilities:
+ Provides training/assistance to assigned individuals with Interpersonal skills and activities of daily living in accordance with the outcomes and goals in the individual’s support plan and under the direct supervision of the Team Leader, Program Coordinator, or Program Director
+ Reports any changes in status to Team Leader, Program Coordinator, or Program Director
+ Documents care accurately per company policies and procedures and turns documentation in on a timely basis. Adheres to client and Amergis policy and procedures at all times
+ Participates in multi-disciplinary team conferences and In-services as requested
+ Provides therapeutic support in daily activities
+ Performs other duties as assigned/necessary
+ Accompanies resident/s to schedule activities both inside and out of the facility/home
+ Participates in contributing appropriate revisions of individual services plans as requested by client, team leader or program coordinator
+ Assists with admissions, discharges and transferring of residents
+ Assists residents with routine household chores and other ADLS to support independence
+ Assists with training new staff if applicable

Length: 13 weeks and then transition to permanent

+ Columbia, Hopkins, and North East Columbia house locations - May be asked to FLOAT
+ Two week PSO at their headquarters in West Columbia - November 11 - 21st (Tues-Friday then Monday-Friday 8-5pm) - NO RTO ON THESE DATES
+ LPNs must have a VALID SC DL
+ Please make sure LPNs are Comfortable driving client's van
+ LPN will convert to perm staff after 520 hours (~13 weeks)
Under the supervision of a Registered Nurse, the LicensedPractical/Vocational Nurse works in a variety of remote or clinic settings toserve a specific population's health and/or wellness needs. These variety of sites include but are notlimited to community or employment immunization clinics, wellness clinics, jobsafety and workers compensation sites and travel health clinics from pediatricto geriatric age populations.
The Licensed Practical/Vocational Nurse demonstrates theability to make clinical judgments in an effective and efficient manner underthe supervision of an RN or Physician. The Licensed Practical/Vocational Nurse demonstrates critical thinkingand performance ability in the coordination of patient care. Consistently performs according to nursingstandards and is accountable in providing safe, quality patient care.
Minimum Requirements:
+ Current Licensed Practical/Vocational Nurse License in statewhere they are practicing
+ One year experience as an LPN/LVN in a clinic settingpreferred
+ Complies with all relevant professional standards ofpractice
+ Current CPR if applicable
+ TB questionnaire, PPD or chest x-ray if applicable
+ Current Health certificate (per contract or stateregulation)
+ Must meet all federal, state and local requirements
+ Must be at least 18 years of age
