    Novant Health is seeking a Clinical Pharmacist PRN role , to join our outpatient pharmacy team, to deliver optimal medication management across all dimensions of care, every time.

    Essential functions include:

    + Facilitates pharmaceutical care services, medication utilization systems within assigned services and care areas to ensure drug utilization activities are aligned with patient care needs, evidence- based best practices and regulatory standards

    + Completes critical patient monitoring and reviews patient profile/ chart to identify, prevent, or mitigate drug- related problems,

    + Partners as an active member of multidisciplinary team and collaborates with healthcare providers within decentralized patient care areas to provide patient- centered care

    + Makes appropriate evidence-based, patient- centered medication recommendations

    + Participates in the management of medical emergencies

    + Provides discharge medication review, reconciliation, and counseling as appropriate

    + Maintains proficiency in and actively engages in central dispensing functions and operational programs

    + Oversees pharmacy operations and technician practice/activities

    + Maintains proficiency in preceptor roles and actively emulates education/ mentorship skills

    Responsibilities

    + Responsible for optimal medication management and promotion of medication safety. Participates in direct patient care activities and rounding opportunities where available and in alignment with department and organizational goals. Monitors therapy for appropriateness, dosage, effectiveness, interactions, and adverse drug reactions.

    + Performs patient assessments. Reviews and acts upon clinical decision support surveillance. Completes organizational and/or departmental required documentation. Participates in patient education and wellness-based initiatives as driven by scope of practice. Prescribes and/or administers medications as allowed by law and scope of practice.

    + Maintains up to date, evidence-based knowledge required for services provided. Reviews and processes orders specific to the population served. Supervises, checks, and controls quality and accuracy of work performed by pharmacy team members. Prioritizes workflow ensuring all services are performed in a timely manner. Maintains and follows formulary approvals, interchanges, stocked medications and non-formulary process.

    Qualifications

    + Education: 4 Year / Bachelor's Degree in Pharmacy, required. Doctorate of Pharmacy, preferred.

    + Experience: Postgraduate year 1 residency or fellowship OR two years of experience as a registered pharmacist in health system pharmacy, preferred.

    + Licensure/Certification: Current Pharmacy licensure in appropriate state(s) or must be completed within 30 days of start date by either examination or reciprocity, required. Board of Pharmacy Specialties Certification, preferred.

    + Additional Skills Required: Other duties as outlined in Novant Health and facility specific policies and procedures. Ability to drive/travel to multiple facilities/locations as needed. Ability to work as part of a high performing, change ready and resilient team.
