    + Help plan safe discharges and transitions of care

    + Coordinate with healthcare teams to keep patients supported and informed

    + Connect patients to community resources for housing, food, transportation, and more

    Job Requirements

    **Education:** Master's Degree (MSW) from a school of social work accredited by the Council of Social Work Education REQUIRED

Our full range of medical services include a new pavilion featuring enhancements in heart and vascular care, surgery, maternity, cancer care, and diagnostic imaging.

    Pay Range

    $24.27 - $36.10 /hour
