Case Manager (Looking to interview and hire ASAP)

About the Role

We are seeking a compassionate and driven Case Manager to support youth in foster care through intensive case management services. This position plays a critical role in coordinating care, building relationships with clients and their support networks, and ensuring the delivery of high-quality, trauma-informed services.

Key Responsibilities

+ Manage a caseload of youth, including those at higher risk of psychiatric hospitalization or intervention.

+ Build and maintain collaborative relationships with clients, families, schools, therapists, agencies, and foster homes.

+ Develop individualized goals and interventions in partnership with guardians and providers.

+ Monitor treatment planning and external service recommendations.

+ Ensure compliance with agency policies and contractual requirements.

+ Participate in on-call rotation and represent the agency in community meetings and task forces.

+ Maintain accurate documentation and timely reporting.

+ Stay informed on trends in human services and case management.

Qualifications

+ Bachelor’s degree in Human Services, Social Work, Psychology, or a related field.

+ LSW or LSWA license (or license-eligible).

+ Valid driver’s license and reliable transportation.

+ Strong interpersonal and communication skills.

+ Detail-oriented with excellent time management.

+ Ability to analyze situations and develop effective solutions.

+ Experience working with children and families from diverse backgrounds.

+ Familiarity with trauma-informed care and behavioral health systems.

Work Environment

+ Office-based with regular travel to client homes and community settings.

+ Noise level is typically low to moderate.

+ Flexibility required for occasional extended hours or weekend work.

The pay range for this position is $48000.00 - $48000.00/yr.
