    Job Status: This position is contract - 10 hours/week

     

    Wage:$21.00/hour

    General Summary of Purpose of this Position:

    Provides home and personal care for elderly, Title III Home Health patients in private residences. Works under supervision of RN Supervisor.

    Primary Job Duties and Responsibilities

    1. Personal Care Services (70%)

    + Explain fundamental hygiene principles and renders bedside care to elderly individuals who are ill or otherwise in need of assistance

    + Assist patient with self-administered oral medications under written direction of physician or as directed by home care nurse

    + Assist patient with ambulation and exercise

    2. Household Care Services (20%)

    + Advise and assist patients on dealing with household tasks, such as managing household utilities, planning nutritious meals, purchasing and preparing foods

    + Perform variety of miscellaneous duties as requested such as obtaining household supplies and running errands

    + Purchase, prepare, and serve food for patient

    + Change bed linens, wash and iron patient's laundry, and clean patient's quarters

    3. Record Keeping (10%)

    + Maintain records of services performed and of apparent condition of patient

    + Participate in evaluating needs of patients, and confers with Caseworker to plan for continuing additional services

     

    Preferred Qualifications:Knowledge, Skills and Abilities that are preferred for success in this position

    Education:

    + Graduation from high school or GED is desired

    Experience:

    + At least 6 months of experience in homemaker and personal care field is preferred

    Confidentiality:

    This position requires regular handling of sensitive and confidential client and agency information. Unauthorized disclosure could harm client interests and the organization, and may also violate state and federal privacy laws. This position may have access to reports, records, and personnel files, and must exercise the highest level of discretion, judgment, and confidentiality at all times.

     

    Knowledge, Skills and Abilities

    Language skills:

    + Read and comprehend basic instructions and present information to others

    Safety:

    + The person in this position complies with HRDC safety policies, procedures & guidelines; participates in safety training & drills as required; Reports unsafe conditions, incidents or near misses immediately; takes proactive steps to ensure the work area is clean and safe at all times.

    Mathematical/ Money Handling Skills:

    + Basic counting skills

    + Add, subtract, multiply, and divide simple numbers

    Reasoning Ability:

    + Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form

    + Ability to deal with problems involving several concrete variables in standardized situations

    + Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form

    + Ability to define problems, collect data, establish facts, and draw valid conclusions

    Computer Software and Operating Systems:

    + Utilize computer Internet/ E-Mail to access, send, retrieve, save, print or transmit documents, or data files

    + Operate Personal Computer with Windows operating system to start programs, input, format and edit data files and print or transmit data

    Specialized Tools (Non-Computer):

    + Household cleaning equipment, such as vacuum cleaner, dishwasher, etc.

    Agency, Professional or Governmental Policies and Regulations:

    + Agency policies or procedures (List by Name): All HRDC policies and procedures

    + Specialized professional or industry guidelines/standards: Title III Home Health Policies

    Driver’s Licenses Required for Performing this Position:

    + Valid Montana Driver’s License

    + Must be bondable

    Professional Licenses or Certifications Required by Law, Rule or Regulation for Performing this Position:

    + Successful completion of Personal Care competency test

    + Must successfully pass annual TB test

    Licenses or Certifications that would be useful but are not required:

    + CAN

    + CPR/First Aid

    Other Required Knowledge, Skills or Abilities:

    + Ability to effectively work and communicate with a wide range of individuals from all socio-economic groups

    + Ability to read, interpret and implement documents such as governmental regulations, rules, and procedure manuals

    + Ability work with minimal supervision and effectively manage work flow

    + Ability to respond effectively to the most sensitive inquiries or complaints from customers or claimants

    Essential Physical, Mental and Emotional Requirements of this Position:

    The following physical, mental and emotional abilities are considered essential to perform the duties listed above:

    + While performing the duties of this job, the employee is frequently required to stand; walk; sit; use hands to finger, handle, or feel; reach with hands and arms; climb or balance; stoop, kneel, crouch, or crawl; talk and hear.

    + The employee must regularly lift and/or move up to 10 pounds, frequently lift and/or move up to 25 pounds, and occasionally lift and/or move up to 50 pounds.

    + Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.

    + The incumbent must be able to perform multiple tasks simultaneously, must be able to work under time and/or deadline pressures.

    Supervision Received:

    + This position operates under limited supervision. The incumbent’s work is primarily assigned by daily workflow and operations. The incumbent is authorized to make decisions as to the timing and sequence of work and the work is performed according to established policies and procedures and/or within applicable laws, rules and regulations governing the tasks being performed.

    This position has the following supervisory responsibilities:

    + Decisions, final recommendations, and/or errors primarily affect clients of one program.

    Judgment Required to Make Decisions:

    + Requires judgment to solve day-to-day problems, but usually within established procedures, guidelines, and precedents

    This position is responsible for the following personal contacts (either in oral or written form) when performing the Duties and Responsibilities of the Position:

    + Daily Interaction with co-workers and patients

    + Weekly interaction with supervisor and Case Worker

    Working Conditions:

    The incumbent is exposed to the following unusual, potentially hazardous or unpleasant working conditions:

    + Works in temperature controlled household environment.

    + Weekly or more frequent operation of a motor vehicle under all weather conditions.

    + Weekly or more frequent exposure to angry or violent clients or volunteers.

    + Weekly or more frequent exposure to risk of cuts, burns, shock or other minor injuries (not requiring hospitalization).

    + Weekly or more frequent exposure to occupational diseases (Specify): exposure to contagious diseases.

    + Weekly or more frequent exposure to unpleasant substances, odors, or noises.

    + Must be able to travel out of town to clients’ homes on as needed basis.
