    THE POSITION

    Take your nursing career to the next level by helping improve the health of your community! The Department of Health is seeking a caring and hardworking Community Health Nurse to provide education and clinical services in the Northcentral District. By working with partners in the community, you will help us make sure Department of Health programs are available and relevant, as well as investigate threats to public health.     DESCRIPTION OF WORK

    In this position, you will be responsible for planning, implementing, and evaluating public health services and initiatives, as well as providing direct public health services and community liaison activities. Your work will involve analyzing county level assessments to identify public health needs, linking program resources with community partnership activities, and developing public health initiatives to address identified health priorities. You will participate on district and local health committees as a representative for the Department of Health to advocate and support the development of public health initiatives. Your duties will include monitoring community health programs, developing and providing health education programs, and providing case management services to promote patient health and safety. Additionally, you will provide clinical services and provide consultations to individuals, health professionals, and community partners.

    Work Schedule and Additional Information:

    + Full-time employment

    + Work hours are 8:00 AM to 4:00 PM, Monday - Friday, with a 30-minute lunch.

    + Work infrequently requires statewide travel and irregular/flexible work hours including overnights, weekends and holidays in accordance with policy.

    + This position will be headquartered in either the Potter County State Health Center or Tioga County State Health Center depending on the location of the selected candidate. If the candidate is headquartered in Tioga County, travel will be required to Potter County 1-2 times per week and as determined by the supervisor.

    + Telework: You will not have the option to telework in this position.

    + Salary: In some cases, the starting salary may be non-negotiable.

    REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY

    QUALIFICATIONS

    Special Requirements:

    + All positions require active authorization to practice as a Registered Nurse in Pennsylvania.

    + All employees possessing an active temporary practice permit must obtain licensure as a Registered Nurse within the one (1) year period defined by the Pennsylvania State Board of Nursing.

    + This position falls under the provisions of the Child Protective Services Law.
