    Full Time | Day Shift | Vancouver, WA

    Hourly Range: $24.99 – $33.73

We are seeking a **Referral Coordinator** to join our **Home Health** team in Vancouver, Washington. In this vital role, you’ll ensure patients receive timely, coordinated, and authorized care while supporting our mission to promote healing and peace of mind at home.

    As a Referral Coordinator, you’ll play a key part in managing patient referrals and insurance authorizations for our Home Health services. Your attention to detail, communication skills, and understanding of medical processes will help patients seamlessly access the care they need. You’ll work closely with nurses, physicians, insurance providers, and billing teams to ensure accuracy, compliance, and continuity of care.

    What You’ll Do:

    + Manage incoming and ongoing referrals for Home Health services.

    + Obtain insurance pre-authorizations and reauthorizations as needed.

    + Verify coverage, complete required documentation, and ensure compliance with Medicare and private payor guidelines.

    + Communicate effectively with patients, clinicians, insurance representatives, and billing staff to support accurate and efficient service delivery.

    + Maintain detailed records and documentation in patient medical files.

    + Support cost-effective utilization of services while ensuring patients receive timely care.

    What You Bring:

    + Minimum **3 years of experience** in medical insurance, referral coordination, or a related healthcare setting.

    + Strong understanding of insurance authorization requirements for therapies, procedures, and diagnostic testing.

    + Proficiency in **Microsoft Office** and familiarity with **Horizon Home Care** systems preferred.

    + Education in business, healthcare administration, or nursing preferred.

    + LPN or RN license in Washington State is a plus.
