    Job SummaryUnder the direction of the Chief Medical Officer, Associate Chief Medical Officer for the Medical Group and the Sr. Director of Advanced Practice providers, this position is responsible for the APP practice model in the assigned primary care clinics and affiliated clinical pathways and programs. This includes the oversight of the APPs’ clinical practice. The APP Supervisor will collaborate closely with physicians and administrative practice operational leaders to provide applicable clinical expertise, clinical leadership, and necessary programmatic development for the assigned specialty within the HonorHealth Medical Group.The APP Supervisor collaborates with multiple professionals to support and coordinate the provision/management of patient care. This role will work with physician leadership to develop and execute clinical and programmatic goals aligned with the overall practice and the strategic objectives and initiatives.The APP Supervisor is responsible for ensuring the delivery of high-quality and cost-effective care, based on competency and evidence based clinical practice and research. This role collaborates with members of the healthcare team to provide care appropriate to the age of the patients served in their assigned practice. This position emphasizes health promotion and maintenance, disease prevention, and diagnosis and management of acute and chronic diseases.In addition to clinical duties, the APP supervisor will have programmatic oversight of APPs in collaboration with physician chiefs and organizational leaders. The role will supervise and ensure appropriate coverage, assist with performance evaluations, and establish education and training protocols. The APP Supervisor will assist in assessing staffing needs, participate in recruitment, onboarding, and retention within the practice.The APP Supervisor adheres to and functions within the scope of practice defined by regulatory agencies and the organization. Adherence to evidence-based advanced practices and utilizes a Shared Governance model for decision-making. TheAPP Supervisor focuses on performance standards that act in accordance with the organization's initiatives and scorecard. This role will prioritize and aim to foster and improve patient care quality, improve clinical effectiveness, and nurture a culture of patient safety. This role will focus on both the clinical programs and the people by working to connect with team members, demonstrate care for employees, and work to improve overall team wellbeing.This role will support finance and growth by optimizing services, improving clinical efficiency, and engaging with all the members of the care team to foster collaboration among providers and allied healthcare staff.This individual possesses an ability to work with all levels of staff within a multidisciplinary group, with a significant level of administrative and clinical autonomy. The APP Supervisor is highly motivated and able to work independently, serving as a clinician, supervisor, program leader, and functions as a mentor, educator, and researcher.The position is approximately 30% administrative and 70% clinical. Job ResponsibilitiesClinical Care:

     

    + Practice focused on Patient/Family: direct care practice using a holistic perspective; therapeutic partnerships with patients; expert clinical thinking and skillful performance; reflective practice; evidence-based guided practice; diverse health and illness management approaches. As a provider, the APP performs history and physical exams, order and interpret diagnostic studies, prescribe medications, consult ancillary departments and other specialists, and provide a smooth transition between various levels of care, collaborating with the interdisciplinary team including physicians, pharmacy, and other outpatient services. Core responsibilities minimally include:

    + Diagnose and treat acute, episodic, or chronic illnesses.

    + Prescribe all necessary medications and treatments.

    + Perform comprehensive physical examinations of patients.

    + Develop and implement patient care management plans.

    + Facilitate referrals to other healthcare professionals and medical facilities.

    + Communicate with collaborating physicians or specialists regarding patient care role and responsibilities.

    • Evidence-Based Program/Services Development/Supervising APPs:

    + Will lead or participate in miscellaneous projects and task forces as assigned and in collaboration with physician leadership.

    + Partner with directors for operations in assigned region. Will be involved in APP performance improvement, engagement, wellness, coaching and mentoring, and recruitment/interviewing of prospective new candidates. Shall assist with performing performance reviews with APPs in their assigned region.

    + Will help develop annual quality metrics for primary care, participate in all-provider meetings, and work with CMO/ACMOs/Sr. Director of APPs on corrective action plans for APPs when necessary.

    + Participate in strategy development, financial planning and budget development for the medical group in collaboration and assigned by physician leadership.

    + Will be responsible for clinic rounding for assigned clinics per schedule as assigned by physician and operational leadership.

    + Promote team vision and patient-centered culture.

    + Provide clinical oversight to Advanced Practice Providers (APPs) in assigned regions as needed including signing home health orders, chart reviews, and credentialing reviews. Follow formal process for review and write the clinical response for patient grievances, as necessary.

    + In collaboration with physician leadership, participates in system wide initiatives and process improvement, including policy writing, coordinating pilot projects, or other projects that may affect the entire network.

    Education

    + Master’s degree Nurse Practitioner or Physician Assistant Studies with Board Certification – Required

    + Doctorate Nurse Practitioner or Physician Assistant Studies - Preferred

    Experience

    + 3 years Clinical experience in Primary Care as an Advanced Practice Provider – Required

    + Demonstrated ability to work collaboratively with physicians and other healthcare professionals to develop and implement programs and processes

    + Demonstrated strong organizational and communication skills to effectively manage operations, individuals, and teams

    License and Certifications

    + Nursing/NP or Physician Assistant - RN/NP or PA Certification and Active State Medical License – Upon Hire – Required

    + Drug Enforcement Administration (DEA) Licensure – DEA license – Upon hire – Required

    + Basic Life Support (BLS) – BLS Certification – Upon hire - Required
