We have a seasoned team of therapists who treat patients with compassion and provide high quality care.
Position Summary:
We are seeking an individual who is passionate about rehabilitation, eager to grow clinically, and thrives in a team-oriented environment. The ideal candidate will provide efficient, high-quality care that focuses on improving patient’s mobility and function. The therapist will work closely with an interdisciplinary team to support patient recovery, mobility, and the discharge plan.
Key Responsibilities:
+ Perform evaluations of patients physical and functional status
+ Develop appropriate goals based on clinical findings
+ Educate patients and families to support active participation in care
+ Collaborate with physicians, nurses, care managers, occupational therapists, speech-language pathologists, etc. to provide coordinated, holistic care.
+ Maintain timely and accurate documentation in accordance with departmental and regulatory standards.
+ Contribute to a positive team environment that values respect, empathy, accountability, and innovation.
Qualifications:
**Position Requirements** :
+ Graduate of approved School of Physical Therapy.
+ Licensed as a Physical Therapist in the State of Illinois.
+ Certified in CPR through the American Heart Association.
