    Position Purpose:

    The physical therapist will assume autonomous responsibility for evaluating patient’s need for physical therapy.

    What the Physical Therapist ( PT ) Will Do:

    + Providing treatment and services within the scope of their license, certification or registration and as required by law and regulation.

    + Evaluation and clinical diagnosis of patient conditions.

    + Establish treatment goals utilizing input from patient and their family as appropriateDocumentationremainsin accordance with established internal/external timeliness standards, insurance standards, and reimbursement guidelines.

    + Participates as a part of an interdisciplinary and patient-centered care team which includes the verbal and written exchange of information with other healthcare team members as it pertains to integrate a physical therapy treatment plan

    Minimum Qualifications:

    + Must be a graduate of a Doctorate’s or previously equivalent professional Physical Therapy training program accredited by the American Physical Therapy Association.

    + Required: Bachelor’s in Physical Therapy

    + FSBPT Certification.

    + State of Michigan Physical Therapist License or eligible to obtain licensure prior to hire date.

    + American Heart Association BLS or equivalent
