Job Summary
Serves as the lead for the coordination and project management of Care Gap projects that utilize operational methodologies to drive substantial improvements in quality, efficiency and patient engagement. Provides guidance and direction to the Keystone ACO Partner Organization Clinical Practice Transformation team who will assist with the implementation of action plans developed to improve identified Care Gaps and Quality measure. Provide education and supportive data concerning quality measure benchmarks, status and improvement needs for primary and specialty care providers in multiple practice sites to facilitate clinical guideline adherence and coordinate services specifically related to the patient’s health care needs.
Job Duties
+ Manages the workload of quality data collection and reporting requirement to meet project milestones through task management, consistent follow through, leading and mentoring other team members, supporting the project team, progress tracking and reporting, and creative organizational problem solving.
+ Serves as a resource for team members for training, support, coaching and mentoring.
+ Coordinates age-appropriate learning principles and supports the health care team with education and clinical guidelines.
+ Participates in the design of interventions to support health management programs.
+ Participates in the interview and selection process of new team members and coordinates training and orientation programs for audit staff.
+ Utilizes established education and clinical pathways to support comprehensive and consistent disease management education to close patients’ care gaps.
+ Directs, plans and coordinates regular meetings and follow up at assigned sites to ensure comprehensive and consistent chronic disease management across the service line while providing nursing with up to date education.
+ Directs, plans and implements improvement plans for specific chronic disease management and quality focus areas for each assigned site.
+ Performs site level data review to evaluate prospective and retrospective outcomes for these programs.
+ Maintains reports and documentation of relevant findings during site evaluations.
+ Identifies problems, proposes process improvement strategies and assists the Care Gap in Quality Improvement and reengineering activities including the identification and development of key clinical initiatives to improve quality and workflow to enhance clinic efficiency and patient engagement.
