    Job Summary and Responsibilities

    Under supervision, the Case Manager plans, organizes and manages a patient caseload:

    + Applies concepts derived from management and communications theory, group dynamics, counseling methodologies, and clinical nursing judgment to implement a clinical nursing program or programs

    + Provides direct patient care through use of nursing process, performs established nursing intervention utilizing current clinical knowledge. Initiates and participates in patient, family and staff education.

    + Demonstrates leadership capabilities.

    + Serves as preceptor for orientees and students.

    Job Requirements

    Minimum

    + 1 year of experience as an RN

    + High School Graduate and High School GED

    + Graduate of an accredited registered nursing program.

    + California Registered Nurse license

    + Current BLS certification from the American Heart Assoc

    + Current California driver's license

    + Clean Driving record with proof of liability insurance for automobile.

    Preferred

    + Home health experience preferred.

    Pay Range

    $50.85 - $75.87 /hour
