Job Description :

**AHN Primary Care Institute** is seeking a **Geriatric Medicine Physician** to join our growing team in **Erie, Pennsylvania** . In this role, you will deliver high-quality, person-centered care in a **consultative/co-management** capacity to older adults with complex medical and social needs.

Highlights:

+ Work closely with primary care clinicians and specialty providers, where you will help guide care plans that promote independence, dignity, and quality of life for aging patients.

+ Practice in a supportive, collaborative environment with access to multidisciplinary team-based resources to ensure coordinated, comprehensive care for every patient.

+ Primary outpatient, with potential for limited practice in skilled nursing facilities/assisted living communities.

Qualifications:

+ MD or DO degree from an accredited medical school.

+ **Board Certification in Geriatric Medicine.**

+ Current Pennsylvania medical licensure or eligibility to obtain licensure.

+ Experience in geriatric care is preferred.
