Overview
Status: Non-exempt, Part-time, Temporary (Dec 2025-Mar 2026)
Hours Per Week: 1 3
Schedule: Friday; 4:00 pm – 11:00 pm, Saturday & Sunday; 3:00 pm – 6:00 pm
Scope of Position: Will support families who are experiencing homelessness using a housing-first approach and trauma-informed practices to build trust-based and collaborative relationships. Focus mainly on HMIS Smartsheet data entry and case management. Maintain accurate HMIS records with a minimum data quality score of 90%. Assist in providing demographics, outcomes, and the number of individuals served to complete funder reports as needed. Respond immediately to help guests overcome barriers to accessing services. Advocate for and connect them with crucial resources to resolve their immediate crisis and access housing to end their homelessness. Coordinate with the Greater Hartford Coordinated Access Network (GH-CAN) and attend community meetings and case conferencing.
Responsibilities
+ Assist guests in resolving their housing crisis independently.
+ Assist with maintaining a safe, secure environment.
+ Assist with shelter intakes.
+ Attend CAN meetings to advocate for guests and ensure coordination.
+ Collaborate with CAN housing providers to expedite moves to permanent housing.
+ Collaborate with the McKinney Vento Liaison to ensure children are enrolled in and attending school.
+ Complete all HMIS data entry for the Overflow Program with a minimum data accuracy of 90%.
+ Complete nightly bed check-ins.
+ Deliver services with safety, respect, and effectiveness.
+ Ensure the nightly census is accurately completed.
+ Enter all data from families utilizing winter programs into Smartsheet for tracking.
+ Maintain a caseload of sheltered guests.
+ Maintain a professional and organized workspace.
+ Perform other duties as assigned by the supervisor.
+ Prepare and complete intake packages for guests.
+ Provide case management to guests, which consists of the following:
+ Assist guests with accomplishing goals, becoming “document-ready,” and obtaining housing.
+ Document case management in the guest file through case notes, referrals, and housing plans within two business days of interaction.
+ Meet with each guest within two business days of intake to assess housing barriers, determine service needs, and complete the initial housing plan.
+ Schedule weekly meetings with each guest to review individual housing plans.
+ Update each guest’s housing plan at least once per month.
+ Review all intakes to ensure that all releases of information have been signed and that the intake is complete.
+ Utilize a strength-based approach when working with guests, employees, the agency, and the community.
+ Utilize the Harm Reduction model and the Trauma-Informed approach during the delivery of services.
+ Work to reduce the average length of stay in emergency shelters by increasing the number of guests who exit to permanent housing.
Qualifications
+ Bachelor’s degree in human services or a minimum of a two-year degree in human-related services and two years of related experience required.
+ A minimum of two years of case management experience is required.
+ Ability to establish and maintain professional boundaries.
+ Ability to maintain confidentiality in all aspects of the work environment.
+ Ability to work in a fast-paced environment and work well under pressure.
+ Demonstrated knowledge of community resources, social service agencies, and landlords.
+ Excellent communication skills, both verbal and written, are required. Spanish-speaking preferred.
+ Experience working with at-risk populations or persons experiencing homelessness; knowledge and belief in the “Housing First” philosophy and strategies preferred.
+ Flexible work schedule, including nights and weekend work required.
+ Must maintain a professional appearance.
+ Previous HMIS and case management experience preferred.
+ Proficient in Microsoft Office Suite.
+ Self-motivated and dependable.
+ Valid driver’s license and reliable transportation required.
+ Must complete KeepSAfe training and other required training upon hire.
