    Description:

    ESSENTIAL FUNCTIONS

    Knows, understands, incorporates, and demonstrates the Trinity Health Mission, Vision, and Values in behaviors, practices, and decisions.

     

    Provide comprehensive pharmaceutical care, including interpreting and verifying medication orders, assessing clinical appropriateness, and monitoring patients’ medication profiles for allergies, contradictions, duplicate therapies, interactions, and adverse effects, while maintaining accurate patient profiles.

     

    Collaborate with physicians, nurses, and other healthcare professionals to clarify orders, recommend therapy adjustments, and answer drug information inquiries.

     

    Educate patients and families on medication use, adherence, and safety; conduct Medication Therapy Management (MTM) interviews and document interventions.

     

    Accurately prepare, compound, and dispense medications, entering orders into the pharmacy system with proper documentation, maintaining records of controlled substance disbursements, and monitoring compliance with dosing guidelines and drug protocols, intervening when necessary.

     

    Administer vaccines (after certification) and ensure documentation in IRIS; comply with all state and federal pharmacy laws and regulations.

     

    Provide oversight and accountability for pharmacy technician activities, including verifying medication orders they prepare and reviewing audit results to ensure accuracy, safety, and compliance.

     

    Ensure adequate inventory of pharmaceuticals and supplies and perform unit inspections in assigned areas to maintain compliance and operational efficiency.

     

    Accurately bill prescription claims to third-party payers and manage order compilation and invoicing for provider offices and EMS services.

     

    Participate in specialty pharmacy care management and quality improvement activities to optimize patient care and operational performance.

     

    Serve as a preceptor for new colleagues and support experiential education for pharmacy students.

     

    Other duties as needed and assigned by the manager.

     

    Maintains a working knowledge of applicable Federal, State, and local laws and regulations, Trinity Health’s Organizational Integrity Program, Standards of Conduct, as well as other policies and procedures in order to ensure adherence in a manner that reflects honest, ethical, and professional behavior.

    MINIMUM QUALIFICATIONS

    Must possess a Bachelor of Science degree in Pharmacy or PharmD from an ACPE-approved college of pharmacy.

     

    Must be a licensed pharmacist in the State of Iowa or eligible for licensure in the State of Iowa. Pharmacist license must be obtained within three (3) months of hire.

     

    Must possess a Pharmacy-Based Immunization Delivery certificate of completion through the American Pharmacists Association (APhA) and qualify for the ability to order and administer vaccinations in the State of Iowa per Iowa Code.

     

    Successful completion of an ASHP-accredited PGY-1 Pharmacy Practice Residency or 3 years of ambulatory pharmacy preferred.

     

    Basic Life Support certified within three (3) months of hire.

     

    Completion of the Iowa Department of Health and Human Services Mandatory Reporter Child and Dependent Adult Abuse training within three (3) months of hire.

     

    Must be comfortable operating in a collaborative, shared leadership environment.

     

    Must possess a personal presence that is characterized by a sense of honesty, integrity, and caring with the ability to inspire and motivate others to promote the philosophy, mission, vision, goals, and values of Trinity Health and MercyOne.

    PHYSICAL AND MENTAL REQUIREMENTS AND WORKING CONDITIONS

    PHYSICAL ACTIVITY REQUIREMENTS:

    **Constant =** 67-100% of workday, **Frequent =** 34-66% of workday, **Occasional =** 33% or less of workday.

     

    Light work: Exerting up to 20 pounds of force occasionally and/or up to 10 pounds of force frequently, or a negligible amount of force constantly to move objects.

    CONSTANT:

    + **Sitting:** Entering medication orders on computer, checking unit dose carts.

    + **Talking:** Expressing or exchanging ideas or information by means of the spoken word, both on the telephone and in person.

    + **Hearing:** Receiving detailed information through oral communication, both on the telephone and in person.

    + **Fingering and Handling:** Input medication orders on computer, pick up medications, medication bags, IV bags, and bottles

    + **Standing:** Filling medication orders, filling unit dose cassettes, mixing IV solutions, filing orders.

    + **Walking:** Picking up medication orders, delivering medications to patient rooms, filing activities in the pharmacy.

    + **Reaching:** Extending hand(s) and arm(s) to remove medications from shelves.

    + **Stooping:** Bending body at the waist while retrieving medications from lower shelves.

    OCCASIONAL:

    + **Climbing:** Ascending or descending stairs by choice, delivering stat medications, or in the event of an elevator malfunction.

    + **Pushing:** Using upper extremities to move carts backward while delivering medications to patient floors.

    + **Lifting:** Raising objects from a lower to a higher position or moving objects horizontally from position to position.

    VISUAL ACUITY REQUIREMENTS:

    + Reading physician orders, computer-generated labels, and fill sheets, labels on products, and other documents.

    + Work extensively with a computer.

    MENTAL/EMOTIONAL REQUIREMENTS:

    + Must be able to set and organize own work priorities, and adapt to them as they change frequently. Must be able to work concurrently on a variety of tasks/projects in an environment that may be stressful with individuals having diverse personalities and work styles.

    + Must possess the ability to appreciate and maintain confidentiality of patient medical status and information.

    + Must possess the ability to comply with Trinity Health and MercyOne policies and procedures.

    EQUIPMENT/TOOLS:

    + Standard medical equipment, special equipment, or office equipment found in the assigned area.

    WORKING CONDITIONS:

    + The worker is not substantially exposed to adverse environmental conditions.
