    SUMMARY OF POSITION:

    Responsible for the collaboration on performance improvement efforts with each nursing unit/educator, serving as resource to nursing in care of the stroke patient. Serve as liaison for prehospital personnel, coordinating meetings and providing feedback on patient outcomes. Maintain awareness of regulatory requirements and facilitate organizational compliance. Serve as coordinator/educator of community outreach activities/collaboration with other disciplines in organizing outreach efforts.

     

    ESSENTIAL DUTIES- The percentage of time spent performing essential functions is 95%. Qualified individuals must have the ability (with or without reasonable accommodation) to perform the following duties:

     

    + Communicate with and serve as a performance improvement resource for the bedside nurse caring for stroke patients.

    + Attend Unit Accountable Care Team and PCNC meetings on nursing units.

    + Review performance measure compliance, log noncompliance, and follow up on performance initiatives.

    + Maintain awareness of regulatory requirements and facilitate organizational compliance.

    + Review and approve patients for inclusion in stroke database.

    + Facilitate chart review, both concurrent and retrospective.

    + Collaborate on preparation and maintenance of patient education materials.

    + Collaborate with data abstractor and data management specialist for data reporting.

    + Coordinate stroke community outreach efforts, collaborating with other disciplines as appropriate.

    + Identify appropriate cases for Stroke program case review.

    + Participate in planning and presentation of educational programs for providers, residents, nurses, prehospital and ancillary staff.

    + Attend local, regional, or national educational programs to maintain current with stroke knowledge.

    + Serve as liaison for prehospital personnel and referring hospital personnel.

    + Contribute to the planning of Stroke Oversight Team meetings.

    MINIMUM QUALIFICATION(S):

    + Registered nurse with a bachelor’s degree required.

    + Minimum three (3) years of experience in nursing required.

    + Currently licensed to practice as a registered nurse by state of employment or holds a multistate RN license through the interstate Nurse Licensure Compact.

    PREFERRED QUALIFICATION(S):

    + Preferred nursing experience in neuroscience, emergency department, or quality

    + Stroke Certification (Examples include: ASC, NVRN, SCRN, CNRN)
