Job Summary
The Diabetes Program Coordinator is responsible for planning, implementing, and evaluating diabetes education programs for patients, families, and staff. This role ensures program content is evidence-based, aligns with current standards of care, and meets regulatory and accreditation requirements, including those of the American Diabetes Association (ADA). The Diabetes Program Coordinator collaborates with interdisciplinary teams to promote diabetes self-management and improve patient outcomes.
Essential Functions
+ Develops, maintains, and updates diabetes education materials and program content based on current clinical guidelines and patient needs.
+ Facilitates individual and group education sessions for patients, families, and caregivers, promoting diabetes self-management and lifestyle modification.
+ Provides diabetes-related education and training to clinical and non-clinical staff as needed.
+ Collects and maintains documentation and outcome data required for ADA accreditation and quality improvement initiatives.
+ Evaluates the effectiveness of diabetes education programs and identifies opportunities for program enhancement.
+ Collaborates with clinical, nutritional, and educational staff to ensure consistent delivery of diabetes education across the continuum of care.
+ Performs other duties as assigned.
+ Complies with all policies and standards.
Qualifications
+ 2-4 years of experience in diabetes education, chronic disease management, or patient education required
+ 1-2 years of experience in program development or coordination preferred
Knowledge, Skills and Abilities
+ In-depth knowledge of diabetes management, patient education principles, and self-care strategies.
+ Strong presentation and communication skills with the ability to educate diverse populations.
+ Ability to analyze and report program data for quality improvement and accreditation.
+ Proficiency in using electronic health records and basic data tracking tools.
+ Strong organizational skills and ability to manage multiple priorities.
Licenses and Certifications
+ RD - Registered Dietician required or
+ RN - Registered Nurse - State Licensure and/or Compact State Licensure required or
+ CDE - Certified Diabetes Educator Certification required
+ BCLS - Basic Life Support required
