Job Summary and Qualifications

Serves as a resource to staff and as a support to the director. Facilitates communication between Care Coordination staff, clinical departments, PTAC, PAS and HIM.

Position Requirements

Required Registration, Licensure or Certification

+ Current Licensure in the State of Kansas as a Registered Nurse (RN) or Current Multi-State Licensure as a Registered Nurse or Licensed Social Worker

+ Current Basic Life Support (BLS) verification course through American Heart Assoc. or American Red Cross - Healthcare Provider CPR

Preferred Registration, Licensure or Certification

+ None

Required Education or Experience

+ Bachelor's degree

+ 2 years of previous experience

Preferred Education or Experience

+ Bachelors of science in Nursing from an accredited institution

+ Three years of clinical experience in acute care or case management

Required Knowledge and Skills

+ Demonstrates ability to communicate effectively

+ Knowledge of accepted program planning and adult education standards and practice

+ Knowledge of accepted Medical Social Services standards of practice

+ Knowledge of laws and regulations affecting the organization

+ Demonstrates excellence in customer service
