**Schedule:** Full Time, 40 hours per week

**Shift** : Monday - Friday 8am - 5pm, rotating weekends, some call

**Location** :

Team Purple: This position will be conducting patient home visits in the Union Park, Conway, Azalea Park, Audubon Park, Vista Lakes area which includes but is not limited to the following zip codes: 32817, 32814, 32803, 32807, 32825, 32813, 32822, 32829, 32812, 32809

**The role you'll contribute:** Provides vocational nursing services under the supervision of an RN, to patients in their homes.

+ Performs nursing skills safely and competently.

+ Performs patient data collection in a holistic manner.

+ Provides accurate and complete documentation.

+ Schedules daily visits to facilitate patient needs.

+ Plans patient care in consultation with Registered nurse.

The expertise and experiences you'll need to succeed:

**Minimum qualifications** :

+ Licensed Practical Nurse or Licensed Vocational Nurse

+ BLS Certificate

+ Valid Driver's License and Auto Insurance

+ One year of medical/surgical experience

Preferred qualifications:

+ One year experience in community health nursing

+ Thirty (30) hours of Florida Board approved IV Therapy Course
