    **Schedule:** PRN

    PRN minimal requirements include :

    4 shifts / month

    2 of which are weekend shifts.

    1 Major / 1 Minor holiday

    **Shift** : Days, in-office, 8a-4:30p

    **The community you'll be caring for** : AdventHealth Palm Coast - 60 MEMORIAL MEDICAL PKWY, Palm Coast, 32164

    The role you'll contribute:

    The Case Management Assistant provides clerical support to the Case Managers and Social Workers to facilitate and execute discharge plans and other related activities as needed. Provides administrative support for the Case Management Department in the absence of department secretary.

    The value you'll bring to the team:

    + Faxing utilization clinical reviews to third party payers

    + Monitoring receipt and distribution of third-party payer requests and logs

    + Assists with locating appropriate facilities for the placement of patients moving to another level of care

    + Facilitation of the transfer to alternate care facilities including transport arrangements Facilitating referrals for home care services

    The expertise and experience you'll need to succeed:

    **Minimum qualifications** :

    + High School Grad or Equivalent

    + 1 years' experience

    + Previous data entry experience
