    Per Diem Hospice Care Advanced Practitioner

     

    Northern Light Home Care and Hospice in South Portland, Maine, is seeking a per-diem, compassionate, and skilled nurse practitioner or physician associate to join our dedicated hospice care team.

     

    In this rewarding role, you will review hospice eligibility, assess patient charts, and provide clinical guidance to nursing staff who perform community-based home visits. While this position is primarily remote, there may be occasional opportunities for face-to-face visits.

     

    You’ll support high-quality care for patients across multiple facilities in the greater Portland area, and the right candidate may reside anywhere in the southern, Central or the Maine Highlands region. Your clinical expertise and heartfelt dedication to patients and their families will make a meaningful difference in their lives during times of greatest need.     Northern Light Hospice Care is a non-profit organization providing direct, personalized care throughout Maine - from South Portland to Fort Kent. We are committed to making visits to those at home who are recovering from illness and surgery and have added Hospice and other programs to help those who prefer to spend their remaining days in the comfort of their own home. We continue with public health nursing by offering immunization clinics, adult health clinics, and education and awareness events for all ages.

     

    Throughout the homecare system, our clinicians make more than 160,000 in-home visits each year caring for an average of 1,400 people on any given day. An additional 4,000 individuals are cared for in health and wellness clinics.
