    Job Description:

    Minnesota Senior Health Options (MSHO) Care Coordinator duties include responsibility for assisting members enrolled in a dual eligible Minnesota Health Care Programs to make informed choices about their health care services, conducting assessments, service coordination, and assisting members in making progress towards his or her health goals and coordinating care with the interdisciplinary team. This includes making in-person visit to enrollees’ home.

    Licensure/Certification Qualifications:

    + Current Registered Nurse License in the State of Minnesota

    + Current MnCHOICES Assessor certification (or ability to obtain within 30 days of hire)

    + Current Basic Life Support certification (or ability to obtain within 90 days of hire)

    + Must have a valid driver’s license
