Description

As a Licensed Practical Nurse in Foster Care, you will coordinate appropriate medical care, dental care, and mental health services nursing care/services to a youth and adolescent population with emotional, behavioral and physical needs. You will ensure that medical treatment services meet the requirements of the regulating agency. You will report to a Registered Nurse and you will work onsite.

Responsibilities:

+ Communicate with medical director/providers regarding observation in client’s health/behavior.

+ Provide client and family education regarding medications and treatments.

+ Collaborate with our staff regarding concerns and behaviors.

+ Participate in Interdisciplinary Medication Review meetings.

+ Nursing documentation using EHR.

+ Update discharge planning needs.

+ See foster children in foster homes as needed.

+ Other duties as assigned within scope of practice and based on agency needs.

Requirements and Education:

+ LPN Licensed in New York State.

+ 1 to 2 years of experience.

+ Experience working with youth and adolescent population desired.

+ Must pass physical.

+ Must have a valid NYS drivers license and reliable transportation.
