    As the HCV Patient Navigation Coordinator, you will play a pivotal role in individuals' lives, making a significant impact through:

    + Review completion of daily temperature checks

    + Review completion of internal controls according to state and agency guidelines

    + Provide Pre and Post-test counseling

    + Perform HCV screening and testing in accordance with state, agency guidelines, and contract guidelines

    + Perform and support Project Safe Points Point of Care testing through the GeneXpert machines

     

    HCV Patient Navigation Coordinator Minimum Salary: $ 58,500

     

    HCV Patient Navigation Coordinator Maximum Salary: $ 58,500

     

    HCV Patient Navigation Coordinator Schedule: Monday - Friday 8:30 AM - 4:30 PM (37.5 hours per week)

    Qualifications:

    + Bachelor's Degree in Health Education or Human Services

    + 1-3 years of relevant experience in Harm Reduction and related fields

    + Valid NYS Driver's License, including a reliable vehicle
