    Candidate must be willing to travel throughout Cattaraugus County on a regular basis.

     

    Grade 307: This is a full time position, working from home, but does require significant travel through Cattaraugus County. The minimum and maximum annual salary that Monroe Plan believes in good faith to be accurate for this position at the time of this posting are $60,949 - $78,362. In addition to your salary, Monroe Plan offers a comprehensive benefits package (all benefits are subject to eligibility requirements) and non-monetary perks.     POSITION SUMMARY

    The Team Lead is responsible for providing coaching, training, and administrative support oversight for the care management staff. The Team Lead is accountable for staff development and performance improvement and for promoting appropriate and effective care through established monitoring systems and professional staff growth. The Team Lead conducts weekly team meetings, manages relationships with hospitals, runs regular case presentations, ensures continuity between fieldwork and meetings, reviews the entire caseload, assists in covering caseloads when needed, promotes model fidelity, staff productivity and staff wellness, monitors quality of documentation, updates resource lists, monitors phase dates, assists in covering caseloads when needed, and manages the caseloads for all staff. This position will monitor and address problems that occur on a day-to-day basis for operational concerns. The Team Lead ensures that the treatment provided is consistent with Monroe Plan philosophy and meets the current standards for quality of care and other requirements as defined by NYS Department of Health (DOH) and Office of Mental Health (OMH). The Team Lead role will require significant multi-tasking in a fast paced, challenging environment. The role will also require field-based work, periodic on-call coverage, and a willingness to work flexible hours.

    ESSENTIAL JOB DUTIES/FUNCTIONS

    Direct Supervision, Coaching/Education Training Staff

     

    + Provide mentoring, guidance and coaching to staff regarding policies and procedures.

    + Provide mentoring, guidance and coaching to staff regarding service delivery and interventions.

    + Assist staff when necessary in areas of Care Management referrals and network development.

    + Promote a positive working environment for staff through constructive reinforcement and recognition.

    + Conduct interim and yearly staff evaluations in collaboration with Manager.

    + Manage recruitment of staff, as well as addressing staff performance-related issues in accordance with salient policies and procedures.

    + Coordinate weekly staff schedules to ensure adequate staff coverage, including on-call for after-hours.

    Department Operational Oversight

     

    + Manage the day-to-day operations in accordance with internal and external regulatory standards and guidelines.

    + Coordinate program outreach during an inpatient hospital admission or emergency department visit with involvement in transition planning, including a needs assessment of community transition supports essential to stabilizing the member.

    + Develop meaningful and lasting partnerships with key community stakeholders including local hospitals and inpatient behavioral health clinics.

    + Facilitate case reviews on “members at risk” with community partners and document recommendations.

    + Manage CTI team’s on-call requirements ensuring that 24/7 crisis response is available.

    + Work with hospital discharge to identify eligible participants and report out on enrollment outcomes and staff assignment.

    + Work with all stakeholders (members, network, families, and government staff) in a caring and respectful manner and with understanding of and consideration for cultural differences.

    Reporting

     

    + Collect and report data, as required and work with team leader and other CTI staff to use data to inform future care delivery.

    + Adhere to program documentation requirements in the Electronic Health Record.

    + Provide program information to members and providers, and other organizations as requested to introduce and support program participation.

    Quality and Compliance Oversight

     

    + In partnership with NYS OMH and regional partners, oversee the creation of a multi-disciplinary Adult CTI community support team.

    + Attend OMH and regional collaboration meetings as needed to adequately support the CTI program.

    + Oversee and/or conduct internal audits/case record reviews on a regular basis to measure compliance to regulatory/contract standards, and report aggregated findings and recommendations to the Monroe Plan leadership team on gaps in care or areas for continued improvement.

    + Assist with the identification, development, implementation, and completion of quality improvement projects that address barriers to care.

    + Ensures that internal and external regulatory agency standards and requirements are met.

    + Monitor, on a regular basis, and no less than monthly, all regulatory compliance areas within the CTI program, including HIPAA.

    + In collaboration with the Monroe Plan leadership team, develops policies/procedures for the CTI program.

    + Support audit process and follows up with staff as needed to address any identified deficiencies.

    Training & Development

     

    + Develop, implement, and monitor training programs within the CTI department as needed.

    + Conduct orientation sessions.

    + Create training materials, as needed.

    + Create testing and evaluation process for staff skill and knowledge following trainings.

    + Plan and conduct continuing education training.

    + Plan the implementation of activities and events, budget spending, material purchase/production and other resources to ensure that resources for education and development are adequate and managed within authorized budget.

    + Act as a liaison between the CTI program and other departments within Monroe Plan as needed.

    + Act as a liaison between Monroe Plan and OMH and community providers and agencies.

     

    Total must equal 100% - essential functions should be completed at least 10% of the time

    OTHER FUNCTIONS AND RESPONSIBILITIES

    + Demonstrate cultural awareness, cultural knowledge, and cultural skills in all aspects of job role.

    + Attend and participate in team meetings and organizational/departmental training.

    + Performs other duties as assigned.

    Position Limitations:

    + Cannot perform any tasks which are governed by license or registration (i.e. cannot answer questions or make recommendations RE diagnosis, medications or treatment).

    + Cannot transport active Monroe Plan members at any time.

    + Cannot perform hands on care.

    MINIMUM REQUIREMENTS/LICENSES/CERTIFICATIONS

    + Licensed Practitioner of Healing Arts (LPHA) – RN, LMSW, LCSW, or Mental Health Counselor.

    + Bachelor’s Degree with 3-5 years post-licensure experience working with individuals who have mental health, or co-occurring mental health and substance use disorders.

    • Minimum of 5 years’ experience in case management or a clinically related area.

    + 2-3 years' previous leadership experience with strong team building and mentoring skills.

    + Knowledge of counseling principles and methods for mental illness and substance use disorders.

    + Knowledge of treatment, rehabilitation, and community support programs as they relate to members, members’ families, and staff.

    + Knowledge of techniques for identifying, assessing, and preventing potentially violent behavior, including crisis management and de-escalation techniques.

    + Ability to develop, evaluate, implement, and modify treatment interventions to meet the needs of individual recipients.

    + Strong leadership and organizational skills required.

    + Excellent planning, organization, and implementation skills required.

    + Must demonstrate excellent oral/written communication skills.

    + Must possess excellent computer skills and have mastery of clinical software required for SOS program documentation and administration.

    + Commitment to our member population.

    + Candidates will need a NYS driver’s license and to own or have access to reliable transportation that enables them to fulfill travel requirements of the job.

    Preferred Qualifications
