Job Summary and Qualifications

Support the physician and interdisciplinary team in facilitating patient care, with the underlying objective of enhancing the quality of clinical outcomes and patient satisfaction while managing the cost of care and providing timely and accurate information to payers. The role integrates and coordinates utilization management, care facilitation, and discharge planning functions. In addition, the Case Manager helps drive change by identifying areas where performance improvement is needed (e.g. day to day workflow, education, process improvements, patient satisfaction).

What you will do in this role:

What qualifications you will need:

+ Minimum Education: Graduate of an accredited school of professional nursing or social work.

+ Minimum Experience: 3-5 years strong clinical experience in clinical practice area, 1-3 years of previous Case Management experience

+ Required Certification: CCMC preferred.

+ (LPN/LVN) Licensed Practical or Vocational Nurse, or (RN) Registered Nurse, or (LMSW/RCSW-I) Lic Master Social Worker/Reg Clin SW-Intern
