    *Physical Therapist (Per Diem) – Putnam Hospital Center – Yorktown/Jefferson Valley, NY*

    *Schedule:* As needed, Monday–Friday, 8-hour shifts between 7:00 AM – 8:00 PM

    *Location:* Yorktown–Jefferson Valley Professional Building, 2585 Navajo Street

    *Overview:*

     

    Putnam Hospital Center is seeking a *Per Diem Physical Therapist (PT)* to join our *Physical Therapy Satellite Department*. In this role, you will evaluate, treat, and educate patients of all ages—pediatric through geriatric—following a medically prescribed physical therapy plan. You’ll work with individuals recovering from injury, illness, or surgery to restore movement, reduce pain, and help them achieve optimal function and independence.

     

    *Responsibilities:*

     

    *

     

    Perform comprehensive physical therapy evaluations and develop individualized treatment plans.

     

    *

     

    Provide hands-on patient care focused on recovery, mobility, and long-term wellness.

     

    *

     

    Document patient progress and maintain accurate records in the electronic medical record (EMR) system.

     

    *

     

    Educate patients and families on exercises, safety, and rehabilitation goals.

     

    *

     

    Collaborate with physicians and interdisciplinary teams to ensure continuity of care.

     

    *Qualifications:*

     

    *

     

    *Required:* Bachelor’s Degree in Physical Therapy from an accredited program.

     

    *

     

    *Preferred:* Master’s or Doctor of Physical Therapy (DPT) degree.

     

    *

     

    *Preferred Experience:*

     

    *

     

    1+ year of professional physical therapy experience.

     

    *

     

    Experience with EMR systems.

     

    *

     

    Background in orthopedics, neurology, or vestibular rehabilitation.

     

    *

     

    *Skills:* Excellent communication, critical thinking, and patient-centered care abilities.

     

    *

     

    *Licensure:* Active *New York State Physical Therapist License* or eligibility for a temporary permit.

    *Working Conditions:*

     

    *

     

    Manual dexterity and fine motor coordination required.

     

    *

     

    Moderate to significant physical effort; potential exposure to typical clinical environment elements (e.g., odors, noise).

     

    *Compensation:*

     

    *

     

    *$67.00 per hour flat rate*
